CCC认证资料提供清单
一．初次申请或相关信息变更时需提供的文件资料
强制性产品认证申请书；
申请人的《企业法人营业执照》或登记注册证明复印件（初次申请或变更时提供）；生产厂的组织结构图（初次申请或变更时提供）；
申请认证产品工艺流程图（初次申请或变更时提供）；
例行检验用关键仪器设备(见认证实施规则工厂质量控制检测要求)清单（初次申请或变更时提供）；
产品总装图、电气原理图；
申请认证产品中文铭牌和警告标记（一式两份）；
申请认证产品中文使用说明书；
同一申请单元内各型号产品之间的差异说明；
同一申请单元内各型号产品外观照片（一式两份）；
需要时所要求提供的其它有关资料（如有CB测试报告请提供）。
二．同类产品再次申请时需提供的文件资料
1．强制性产品认证申请书；
2、产品总装图、电气原理图；
3、申请认证产品中文铭牌和警告标记（一式两份）；
4、申请认证产品中文使用说明书；
5、同一申请单元内各型号产品之间的差异说明；
6、同一申请单元内各型号产品外观照片（一式两份）；
7、需要时所要求提供的其它有关资料（如有CB测试报告请提供）。
3C强制认证产品检测提供文件清单
产品检测送样时应提供以下资料：
送样登记表；
CCC申请详细资料；
产品说明书；
产品规格书；
产品维修手册；
产品电路图（包括原理图和印制刷线路版图）；
同一申请单元中主送型号产品与覆盖型号产品的差异说明；
产品与安全有关的关键元部件明细表和对电磁兼容性能有影响的主要零部件明细表；产品关键安全元件认证证书复印件；
产品的CB测试证书和报告（如有）；
产品的商标使用授权书（如有）；
CCC认证申请步骤
1、向指定认证机构提交意向申请书，应包括如下内容：
申请人信息，制造商信息，如：名称、地址、联系人、联系方式等；
生产厂信息（包括质量体系的状况和体系获证情况等）；
产品名称、型号、规格、商标等；
另外，还需提供产品的相关资料，如： 产品说明书、使用维修手册、产品总装图、工作（电气）原理图、线路图、部件配置图、产品安全性能检验报告、安全关键件一览表等。报关,报检 若申请的产品是电工产品，需做电磁兼容型式试验，还需补充提供下述技术资料
1.电磁兼容性技术标准；
2.可能影响安全性能的产品变更情况说明；
3.产品电磁兼容性能检验报告（按型号提供）；
4.所有的有电磁兼容要求的电磁兼容关键件一览表。报关报检,外汇核销,出口退税,通关咨询
如果所申请的产品是已获证型号产品的变更，或与已获证产品有联系，申请人应在申请书中做出说明。报关报检,外汇核销,出口退税,通关咨询
2、原则上按型号提出申请。不同生产厂生产的同型号产品或同一生产厂在不同地点生产的同型号产品，应分别申请。报关报检,外汇核销,出口退税,通关咨询
3、CCC认证机构会同分包实验室审查申请资料后，划分产品单元，并就检测所依据的标准、检测项目和所需样品数量同申请人达成一致意见后，由分包实验室向CCC报产品检测费用。报关报检,外汇核销,出口退税,通关咨询
4、CCC认证机构会同业务代表根据生产厂的质量体系状况，确定生产厂质量体系的审核天数，并确定相应的体系审核费。
5、CCC认证机构向申请人寄发CCC产品认证报价单。报价单包括认证收费的详细内容、产品名称型号、检测标准和样品数量等。
6.申请人在报价单上签字盖章并返回后，CCC认证机构向业务代表下达任务通知书，委托业务代表与申请人签订认证合同，并向申请人发正式申请书。申请书中包括有需补送的资料、申请方的责任及需附送的《质量体系情况调查表》等。
