各位已登記參加「大學畢業生實習計劃」的畢業生：

在金融海嘯的背景下，政府特別推出具時限性的「大學畢業生實習計劃」，鼓勵企業向2008及2009年畢業的大學生提供實習及就業機會。因應「計劃」的目標和進展，「計劃」接受畢業生提出實習職位申請的截止日期為2010年3月31日。

2.因應上述截止日期，如你欲透過「計劃」尋找合適的實習職位，請你盡快把握最後機會，並在上述截止日期前提出有關申請。請注意：

尊敬的领导：

本人就即将进行的就业实习提出申请，并就有关事宜保证如下：

1、本人确知学院的教学、实训、实习、考试、毕业生信息采集、毕业派遣等过程和时间安排及学分要求，并保证可通过个人努力完成以上各事项。(以下专本通读班学生抄写：)本人保证自愿参加XXXX届专科毕业过程，确认自己能取得合格学分、取得毕业证书及相关毕业手续，并确认由此造成的对本人本科阶段学习及毕业造成的不利影响与学校无关。

2、本人确认已受过系统、完整的毕业教育，通读过《XXXX届XX高校毕业生就业信息网使用及就业指导手册》，完全了解本人在毕业过程中的全部国家、学校政策及相关程序，并保证遵照国家及学校的规定办理，特别保证能及时与就业单位进行网上签约，达成就业意向。

3、本人确认在校期间受过系统完整的安全教育，确切了解就业实习单位的性质、网上签约的资格、岗位职责、相关待遇、安全防护知识及安全操作规程等情况，确切了解在该单位就业后本人的发展前景。本人自愿与用人单位达成就业实习意向，并遵守实习单位的规定，遵守安全操作规程。未经学校和实习单位同意，本人保证不更换实习单位。更换单位后保证在第一时间通知班主任老师。

4、本人保证离校前对本人使用或参与使用的学校公物情况进行了清查，对参与使用的教室、宿舍等场所的卫生进行清扫，因本人清查不明、清扫不力所产生的维修及清扫费用由本人承担。

5、本人确认已将本人实习期的有效联系方式告知班主任老师，并保证此联系方式的更改时能及时通知班主任老师，本人确知班主任的有效联系方式，并保证主动的、经常的与班主任取得联系，实习期间能够经常的浏览学校及机电学院的网站，登陆各班级的QQ群及时了解学校的相关政策和重要通知。

6、本人确认在校期间已受过系统全面的法制教育，在就业实习期间，保证遵守法律法规，不参加非法社团活动及传销组织。

7、本人特别保证在就业实习前和就业实习过程中按学校要求及时上交各项费用和照片，如实填写完成并上交“毕业实习申请表”“毕业实习鉴定表”“毕业实习协议书”“毕业生推荐表”等各种文件，按时参加各种考试和信息采集，按时返校办理正式毕业离校手续。

8、本人特别保证实习返校期间遵守校规校纪，爱惜公物、清扫卫生。

9、本人自愿作出上述保证，并愿意专门就因未能实现保证而导致出现的以下问题由自己承担责任，与学校无关。

(1)、就业实习期间发生的安全事故。

(2)、就业实习期间本人发生的违法乱纪事件。

(3)、未能达到合格毕业、毕业派遣或推荐就业标准，不能取得毕业证书和毕业派遣手续文件。

(4)、就业实习期间为解决关于本人的遗留问题所产生的费用，由本人承担。
