入学申请

如鸟儿渴望蓝天一般，我憧憬着实现梦想的那一刻。十八岁，无限活泼富有激情的年纪。十二年寒窗后，于今年夏天，我迈入了渴望已久的象牙塔，学习播音与主持艺术专业，然而，我向往的蓝天是日本南山大学。

日本本就是一个令人神而往之的国家，关于教育，更是毋庸置疑。如众人所知日本拥有丰富且高水平的大学院教育资源，而且日本的大学院教育是以导师制为最大特色。此外，日本教育包含着对学生自主管理能力的培养，并且在经济地位仅次于美国的日本学习，可以接触到世界领先的高水平东西。南山大学是名古屋一流的私立大学，对学生的英语水平要求高，有助于激发我学习英语的热情。其办学信念为“基于基督教的世界观进行学校教育”，这一点不同于世界其他名流大学，本着这一宗旨，我可以领悟到之前从未接触到的思想。以上就是我希望到日本南山大学学习的原因。

第五篇：申请小学入学情况反馈表

小豆带回了一张《2012年浦东新区小学招生入学须知》，大意是“义务教育阶段实行公办学校就近免试入学制度，将根据孩子在浦东新区的常住户籍地址，对口安排进入一所公办学校”，并要求填写一张“小学升学情况反馈表”，看了一下选项，勾选了“户籍信息没有变化，按户籍地址对口入学”。

4月2日，招生办网站公布对口学校；5月13日前，会收到孩子就近对口的《入学告知书》；5月19、20日可持《入学告知书》到相关对口学校报名；8月15前将会发放录取通知书。

原先本来左思右想想让小豆上个好一点的小学，但是考虑到接送的问题，大人小孩都比较疲劳，还是选择就近入学吧！也不期望小豆学习成绩有多么好，中上水平就可以了，健康快乐比较重要。

附：《2012 年浦东新区小学招生入学须知》

尊敬的家长：

您的孩子即将跨入小学阶段学习，现根据上海市教育委员会关于义务教育阶段入学规定，特向您告知本区小学招生事宜：

1.本市义务教育阶段实行公办学校就近免试入学制度，我们将根据您孩子在浦东新区的常住户籍地址，对口安排他（她）进入一所公办学校。常住户籍必须符合以下条件：户主须是父母或祖父母、外祖父母，并具备相应的房产证。如果学生本人是户主，须提供学生本人为产权人（全部或共同共有产权）的房产证。户籍的最后统计日为2012 年6 月30 日。

2.如属“人户分离”，需按房产证地址对口入学，房产证上的产权人须是学生父母或学生本人，并共同共有全部产权。在入学安排时，需待区域内有户籍同时有房产证的学生安排后，视学校招生情况，统筹安排公办学校。廉租房参照本条办理。

3.持人才引进类《上海市居住证》，有浦东新区房产证，产权人须是父母或学生本人，并共同共有全部产权，可按房产证地址对口入学。无房产证，可根据实际居住地址，视学校招生情况，统筹安排公办学校。

4.如果您孩子的户籍所在地遇到动迁，户籍尚未迁移到新住址，可凭动迁协议书安置地址对口入学（租赁房屋地址只能统筹安排）。

5.本市外区县户籍的幼儿园毕业生，如需继续在浦东新区就读，须在随后下发的《反馈表》中表明要求，我们将根据实际居住地址，视学校招生情况统筹安排。

6.非本市户籍的幼儿园毕业生，希望继续在浦东新区借读，须在随后下发的《反馈表》中表明要求，我们将根据实际居住地址，视学校招生情况，统筹安排公办学校。

7.5 月13 日前，您将收到孩子就近对口的《入学告知书》。

8.请于 5 月19、20 日请持《入学告知书》到相关对口学校报名。

9.如果您的孩子要选择浦东新区教育局批准招生的民办学校就读，请于 2012 年5月初网上报名，5 月19、20 日民办学校组织面谈，被预录取的学生将《入学告知书》交相关学校，以示放弃公办对口学校，同时作为民办学校的录取依据。

10.我区将在8 月15 日前向家长发放全区统一印制的，盖有浦东新区招生办公室和录取学校公章的录取通知书。
