“单独两孩”这样办证

符合单独两孩政策申请生育第二个子女的，需要提交哪些材料？

1.双方身份证、户口簿、结婚证；

2.《再生育申请表》和双方近期免冠两寸照片；

3.一方是独生子女的有效证件即独生子女父母所在单位或户籍所在地乡（镇）街道出具其生育子女情况的证明、独生子女父母光荣证（独生子女证）或申请人承诺声明。视为独生子女的应提交如下的证明：（1）父母生育的子女死亡的，需提供发生子女死亡所在医疗机构、注销户籍的派出所或户口所在地村（居）委会出具的子女死亡证明材料。其他子女达到法定婚龄后死亡的，村（居）委会应出具其未生育或收养子女的证明。（2）收养的以1992年4月1日起实施的《中华人民共和国收养法》为界限，在此之前收养的，由乡（镇）人民政府或街道办事处出具收养证明；在此之后收养的，应提交民政部门出具的《收养证》。单独夫妇一方为本省户籍、一方为外省户籍的，外省一方应提供户籍所在地乡（镇）人民政府或街道办事处出具的婚育证明。独生子女证丢了怎么办
