7.护照或通行证号码:进出境有效证件的号码。
8.居留证件号码:有效期一年期以上居留许可证号码或华侨、港澳同胞暂住证号码或台湾居民居留签注号码等。
9.身份证件号码:外国(地区)企业常驻代表机构工作证、外国人就业证、外国专家证等证件的号码(中国驻外使领馆馆员填写驻外使领馆人员身份证明号码)。
10.物品批文号:填写相关部门批准进出境禁限物品的批准文号,无禁限物品的免填。
11.联系电话:境内联系电话。以下内容按提货单填写
12.进/出境:进境、出境、其他(写明原因,如报废等)。13.进出境口岸:物品实际进出我国关境口岸海关的名称。
14.启运/运抵国(地区):启运国(地区)指进口货物直接运抵我国的起始发出的国家(地区);运抵国(地区)指出口货物离开我国关境直接运抵的国家。
无实际进出境的,本栏目填报中国。请按海关规定的《国别(地区)代码表》选择填报相应的起运/运抵国(地区)的中文名称。

29.受托方名称及海关代码(身份证件号码):委托代理公司申请的,填报代理公司名称及海关代码;委托他人申请的,填报受托人姓名和身份证件号码。以下内容按装箱清单、发票填写: 30.项号:物品序列号。
31.物品税号:按《入境旅客行李物品和个人邮递物品进口税税则归类表》归类填写。
32.物品名称:物品规范的中文商品名称。汽车申报为:车辆牌名(如捷达、佳美等)+排气量(以“CC”为单位)+车型(如越野车、小轿车等)+车驾号,旧车应在车辆牌名前加“旧”字,新车不加。
33.规格/型号:如普通电视机应申报荧幕尺寸,冰箱应申报容积数,汽车申报型号,如E320、750I等。
34.数量:按申请物品的实际数量填写。如电脑主机1台、CD片20片、家具5件。
35.单位:按法定计量单位填写。36.币制:按发票币制填写。37.总价:该项物品的总价。
38.申请人签名:由申请人本人按进出境有效证件签字样式签名。39.机构签章:申请人所在机构公章。40.申请日期:向海关呈交申请表的日期。41.附页数:附页总页数(无附页的填“0”)。

23.物品名称:物品规范的中文商品名称。汽车申报为:车辆牌名(如捷达、佳美等)+排气量(以“CC”为单位)+车型(如越野车、小轿车等)+车架号,旧车应在车辆牌名前加“旧”字,新车不加。
24.规格/型号:如普通电视机应申报荧幕尺寸,冰箱应申报容积数,汽车申报型号,如E320、750I等。
25.数量:按发票所列数量填写。26.单位:按法定计量单位填写。27.币制:按发票币制填写。
28.总价:该项物品的总价值,须与发票一致。

第四篇:进出境自用物品申请表
一、业务流程
常驻人员递交申请→预录入→海关审批→签发《进出境自用物品申请表》→常驻人员报关进口→办理车辆有关手续→归档。
二、需要提交的单证
常驻人员申请进境自用物品,应当提交下列单证:
(一)身份证件;
(二)长期居留证件;
(三)《中华人民共和国海关进出境自用物品申请表》(以下简称《申请表》);
(四)个人申请;
(五)所在单位任职证明;
(六)劳动就业证(或专家证)正本及复印件;
(七)企业批准证书复印件;
(八)企业营业执照复印件;
(九)发票及装箱单复印件;
(十)自理报关单位注册登记证明书复印件;
(十一)法规摘要;
(十二)海关认为必要的其他单证。
办理机动车辆牌照申领手续,应提交下列单证:
1、进口车辆查验登记表;
2、进出境自用物品申请表复印件;
3、《进口机动车辆随车检验单》复印件;
4、进口货物报关单及税单复印件;
5、车辆挂牌申请;
6、海关认为必要的其他单证。
三、海关工作时限:对符合政策规定、单证齐全有效的进境自用物品申请和机动车辆牌照申领手续,5个工作日完成审批。

外国专家进出境自用物品海关手续问题的通知

为了进一步完善自去年11月实施的新的旅检现场申报制度,加强后续管理,规范有关作业程序,决定自1993年11月15日起,外国专家申请进出境自用物品的海关手续按照(84)署行字第325号《关于实施〈海关对外国企业、新闻等常驻机构和常驻人员进出口物品的管理规定〉的通知》及监二
(一)〔1991〕770号《关于启用新修订的〈进出境自用物品申请表〉和〈进出境办公用品申请表〉的通知》中有关条款规定办理,即:有关专家进出境自用物品应事先填具“进出境自用物品申请表”,向其所在地海关提出申请,经批准后,有关物品方可进出境。外国专家现有免税待遇及申请进出境自用物品所需有关证明不变。
总署(84)署行字第206号和(84)署行字第596号通知中有关外国专家凭批有“引首次”和“专首次”字样的“旅客行李申报单”,办理进出境自用物品海关手续的条款规定予以废止。
