安全、文明、绿色施工现场会汇报资料
各位领导、各位同仁：
大家下午好！
衷心感谢各位在百忙之中抽出宝贵时间莅临国宾总部基地项目工程！特别感谢成都市建设工程施工安全监督站领导对国宾总部基地项目工程安全工作的指导！
成都市工业设备安装公司具有机电安装、房屋建筑、市政公用工程总承包一级，化工石油工程总承包二级，消防设施、建筑智能化施工、机电设备安装、装饰装修、防腐保温、钢结构工程专业承包一级；具有压力管道、起重设备、锅炉、电梯等特种设备安装/维修/改造许可证。公司注册资本1亿元，资产总额11.8亿元，年施工能力达30亿元以上；公司开拓全国及国际市场，在云南、天津、北京、贵州、重庆、广东、海南等地设置10个分公司，并参与了北也门、索马里、巴基斯坦等国家建设项目的施工，公司先后荣获“夫蓉杯”、“天府杯”、“蜀安杯”、“中国安装之星”等优质工程奖70项，两次荣获“全军优质工程”一等奖，五次荣获“中国建筑工程鲁班奖”。我代表成都市工业设备安装公司、国宾总部基地项目部对工程情况作如下三点汇报：
一、工程概况：
国宾总部基地项目由成都必喜食品有限公司投资建设、四川飞红工程管理咨询有限公司负责本工程监理，成都市建设工程施工安全监督站安全监督，成都市工业设备安装公司总承包。建筑面积140440m2，7栋框剪结构的科研用房，建筑层数6-15层。2011年11月18日开工，2014年9月20日竣工。
二、从“四节一环保”管理出发，确保安全、文明、绿色施工
（一）制定目标如下：
质量目标：优质结构，一次性验收合格；文明工地目标：创市安全文明标准化工地；安全目标：重大事故为0，轻伤频率为小于0.3%;绿色施工目标：积极响应按照四川省建设厅、成都市建委对绿色施工工程的规范、标准、规程规定实施。
（二）、安全、文明、绿色施工组织机构
1）、以公司与项目部相结合，公司成立安全、文明、绿色施工领导小组对项目部进行指导，项目部成立安全、文明、绿色施工实施小组组织实施，并设置专职安全、文明、绿色施工管理员。
2）、项目部明确安全、文明、绿色施工职能分工，明确奖罚措施，权、责、利、相结合，责任落实到人。
（三）安全、文明、绿色施工的管理 成都建工---成都市工业设备安装公司
国宾总部基地项目
在施工中，施工组织设计、专项方案、应急预案先行。编制适用本工程的施工组织设计、专项施工方案，安全防护和文明施工计划，专项安全施工方案。对特殊部位的高支模部位的支撑安全防护进行了专家组论证。特别对安全措施费投入方面遵照市建委、市安监站的文件规定要求做到详细周密的计划全额投入，用于购置安全防护和劳保用品。本工程搭设脚手架用的钢管壁厚、扣件重量，顶托外径均抽査10件现场检验合格，防护用品都作了进场质量检查和受力试验，坚决杜绝不合格材料入场。确保专款专用。
建立健全项目管理安全生产责任制，明确目标和职责范围，严格考核制度。按照市安监站安全文明施工图集，针对施工环境特殊和施工现场狭小施工作业的施工特点，项目领导班子对安全、环保工作的要求特别重视，谁主管生产，必须亲自抓安全，责任到人，分工明确，把安全生产放在第一位。随着施工进度的不断进行，我们作为总承包单位对总包管理进行了全面细化，各专业工种、分包单位签订了安全文明施工责任书。新进场的所有员工均进行了入场三级教育、岗前专业技能和安全操作规程培训和职业健康教育，分部分项工程施工前对分管工长和班组长进行书面安全技术交底并做好记录和签字手续。，做到了先教育、先培训、后上岗，并积极推行班前活动。每周组织专项安全检查，定期召开安全例会，研究解决问题，总结经验，克服困难。
三、施工中从以下6个方面采取措施确保本工程达到市标准化工地
1、员工健康与安全
现场设立医务室、农民工技校、易燃易爆库房。
2、环境保护
在主要出入口、办公区设立环境保护标识牌、宣传牌。建筑垃圾入箱，分类收集。施工作业区和办公区分离布置。裸土采取塑料布覆盖减少扬尘。产生扬尘的施工作业区采取遮挡措施。现场进出口采用冲洗装置洗车、降尘。办公区废电池、墨盒、废纸封闭回收，垃圾装袋分为“可回收”“可不回收”两类。木工棚采用封闭式降低噪声。
3、节材与材料资源利用
临时设施采用可回收材料。采用落地式和悬挑脚手架。建筑木料合理利用，短木方接长重复利用，板材下脚料制作后浇带盖板、踏步护角，垃圾箱。混凝土剩余料做预制暗盖板。办公用纸分类摆放、纸张双面使用。现场临建设施、安全通道、防护栏杆采用定型工具化搭设，可拆迁、可回收。钢筋余料制作马登筋及定位筋。
4、节水与水资源利用
签订劳务合同时，将节水指标纳入合同条款，专人进行用水记录考核。现场办公区，生活用水与工程用水分别计量，各栋楼设置专用水表并建立用水台账。混凝土养护采用薄膜覆盖、蓄水养护。办公区厕所采用节水器具。
5、节能与能源利用。
实行用电计量管理，控制施工的用电量，办公区与施工区用电分别计量，各 2 成都建工---成都市工业设备安装公司

主要耗能设备严格计量，收集用电资料，建立节电统计台帐，提高节电率。设立机械设备技术档案，定期进行设备维护保养，并有记录台帐。夜间、施工现场采用节能照明灯和节能路灯。
6、节地与土地资源保护
临时设施按二层设计轻钢活动房。劳务工人无生活住房，在外租住，现场减少临设用房。
总之，在本工程整个施工过程中，我们受到了成都市、金牛区两级安监站对施工现场全过程进行全面监督管理，集团总公司、公司各级领导职能部门均定期组织了对施工质量、进度和施工现场进行安全文明施工进行综合大检查，对工程施工安全、环境保护、职业健康、文明施工、扬尘治理等各项工作给予了及时指导和建议，项目经理部安全文明施工领导小组及时进行检查、整改、提高、完善。本工程从开工到现在没发生过一起安全事故，受到甲方和监理单位的好评。
经市、区安监站领导多次莅临现场指导，在监理单位、建设单位驻现场检查指导，公司领导高度重视，项目部全体管理人员齐心努力下，本工程在2014年4月5日顺利通过市标准化工地的现场验收。工程质量合格，优质结构。
综上所述，以上成绩取得与上级领导关怀和各位同仁支持是分不开的。我们决心在成绩面前不骄傲，困难面前不气馁，善始善终，优质保量的完成国宾总部基地项目的施工建设任务，为业主单位交一封满意答卷。

尊敬的各位领导各位专家： 你们好！
首先对你们的到来表示最热烈的欢迎和最忠心的感谢！
一、工程概况：
国宾总部基地项目1#-7#楼工程施工位于成都市金牛区郎家村3组，土桥村5、12组； 施工现场四周道路畅通，交通方便，附近几家工地正在进行建筑物地下室施工，四周场地空旷，旁边有中铁八局办公楼、迎宾一号酒店和迎宾国际；建筑红线内，施工场地狭窄。
本工程为国宾总部基地项目工业性科研用房工程。建筑总面积：140439.99m²，地上面积：约106190.274 m²，地下面积：约34249.72 m²。建筑结构类型：框架剪力墙结构，基础类型：柱下独立基础、条形基础、局部筏板基础。建筑层数：
1#搂地下1层、地上8层，层高4.90-4.20m，建筑高度：37.30mm。2#楼地下1层、地上9层，层高4.90-4.20m，建筑高度：41.50m。3#搂地下1层、地上6层，层高4.90-4.20m，建筑高度：29.50m。4#搂地下1层地上8层，层高4.90-4.20m，建筑高度：42.10m。5#搂地下2层地上8层，层高4.90-4.20m，建筑高度：38.50m。6#搂地下2层、地上12层，层高4.90-4.00m，建筑高度：54.80m。7#搂地下2层、地上15层，层高4.90-4.00m，建筑高度：64.80m。建设单位：成都必喜食品有限公司 设计单位：成都市基准方中建筑设计所 监理单位：四川飞红工程管理咨询有限公司 施工单位：成都市工业设备安装公司 安全生产许可证号为:AJ-11-201 开工日期：2011年11月18日

二、项目部组织管理程序
本工程自开工之初，就确保“创市安全质量标准化示范工地”的目标，项目部为确保目标的实现，组建了以项目经理为主，各管理人员和班组长参加 的安全领导小组，防火领导小组，综合治安领导小组等。安全领导小组是项目安全生产最高权利机构，负责项目安全生产重大事项的决策和过程管理，通过以上各机构的运转，全面带动了整个现场的安全生产和文明施工工作。项目部按照四川省、成都市建设行政主管部门文件要求落实和整改。在施工过程中，我项目部重点加强过程控制，使国家的法律、法规、公司的各项安全管理制度得到严格贯彻。项目部设置了民工技校，定期对工人进行三级安全教育和根据施工过程阶段性、不定期对工人进行专项安全教育，确保特殊工种人人持证上岗。安全管理人员每天对安全设施进行检查。项目部编制了施工组织设计，土方开挖方案，脚手架方案，临时用电方案，高大模板及模板专项方案、物料提升机专项方案、文明施工方案等，在施工过程中严格按照方案施工。
三、现场容貌：
1、施工现场出入口及工程全貌
施工现场出入口严格按照公司CIS形象标准设置工地大门，大门侧边是值班室，24小时有警卫值班。施工现场设置了五牌二图及宣传栏，随时对工人进行安全生产教育、及时宣传安全生产知识。工地四周围墙上附有警示字画及文明标语，不仅起到了文明宣传作用，也使工地整体显得美观、整洁、大方，更是为工地增加了一道亮丽的风景线。
2、现场的平面布置情况
1、现场的围挡，作业区、办公区、生活区合理布置。道路两侧，材料堆放场地及脚手架四周设置连续、通畅的排水沟，并定期每周清理一次，材料堆放场地及脚手架四周设置连续、通畅的排水沟，并定期每周清理一次，同时项目部安排专人进行保洁确保现场排水流畅，无大面积积水，施工现场做到工完场清，及时搞好落手清工作。材料物品堆放整齐划分、标识清晰，各危险源处醒目的安全警示牌齐全。现场设有吸烟室、茶水亭，给工人一个短暂的休息空间。施工现场设施齐全，专人保洁。
3、结构施工阶段
脚手架-----根据工程特点，该工程采用的是悬挑式和落地式扣件钢管脚手架，方案经过监理单位、建设单严密审批，严格按照施工方案搭设。每次悬挑高度在20m以内，悬挑槽钢水平梁挑出部分用黄黑色调和漆相间图刷，在悬挑楼层与挑梁间的间隙处用木板及竹笆封闭严密，外侧张挂绿色安全密目网封闭施工。竹笆隔层满铺。连墙件按两步三距设置，与外架连接牢固。剪力撑采取连续式设置。外架横平竖直、牢固美观、整洁。钢管、扣件及密目式安全网合格证及检测报告齐全。做好三步一验收记录，验收合格后方可使用。
大型施工机械----该项目1#-7#楼主体结构施工采用1#-7#共7台塔式起重机。作为主体结构施工垂直运输工具；塔式起重机均由具有相应资质的装拆公司进行安装，安装完成，由特种设备检测部门检测合格后，通过联合验收并完成备案。基础四周用钢管搭设防护栏杆，并用调和漆涂刷成警示色。目前4#塔机巳拆除，1#5#楼塔机已报停。室内二次结构砌填充墙和装饰工程施工垂直运输工具：1#-6#楼安装龙门架，1#3#4#5#楼龙门吊正在使用，2#6#楼正准备安装，7#楼正准备安施工升降机。
中小型施工机具----中小型施工机具是安全生产的重要方面，施工机械实行定人、定机，持证上岗，严格按照规程操作。项目部定期对机具操作工进行岗位培训。机械操作人员认真负责。按照机械操作规程要求，对所有机械设备安排专职机修工进行日常的检查、保养和维修，确保机具运行正常，安全可靠。
施工用电------现场临时用电采用TN-S系统，严格按专项方案进行实施。施工用电系统采用“三相五线制”。做三级配电，三级保护，末级保护采用漏电动作电流不大于30MA的漏电断路器。施工机械用电实行“一机一闸一箱一保险”。施工现场的每台设备都有自己专用的开关箱，箱内闸刀（开关）及漏电保护器完整齐全。
四、安全防护措施
“三宝四口”-----现场使用的安全帽、安全带、安全网均为合格产品，并附有合格证。为防止高处坠落事故的发生，项目部对预留洞口、临边进行了严密的防护，防护栏杆均涂刷黄黑警示色。楼梯等临边采用防护栏杆防护，及时跟随到施工层，有专人负责，设专人督查。1.5平方米以内的洞口均用固定盖板防护，盖板上表面刷黄、黑相间的警示色。1.5平方米以上的洞口四周设围护栏杆和踢脚板，并隔层用盖板封闭防护。在通道口、物料提升机搭设了双层防护棚，下方铺设安全网，有效预防物体打击事故的发生。
五、生活区
职工宿舍为活动板房，结构安全，整齐干净。宿舍内统一配置了高低床、桌椅、储物柜等生活设施;工地浴室、厕所按要求铺贴地砖、瓷砖、通风良好，定人定时进行清扫，保持卫生。食堂、餐厅地面为地砖，墙面、平顶均为彩钢板。厨房用带纱门窗，清洁明亮，食堂厨房内配备蒸饭器具、生熟食分别存放、配有冷冻、消毒设备及灭蝇设备。炊事人员有健康证明。并设置了文化活动室和医务室，定期对工人进行体检，关心职工的身心健康，丰富职工的业余生活。

Wang Xiao-yu just graduated from the South-west Petroleum Institute, and was assigned to be the practice technician of No.17 Great Wall Drilling Crew.王小雨刚从西南石油学院毕业，分配到长城17钻井队当实习技术员。
It was the first day that he reported to GW17 today.今天是他到长城17队报到的第一天。
He met Mr.Chen Ming, the driller who came from the rig floor, at the well-site.在井场,他遇到了从钻台走下来的司钻陈明。
He went to him and asked: “Excuse me, Sir.My name’s Wang Xiao-yu, a new practice technician of this Crew.”
于是他走了过去，询问道：“你好！我是刚分来的实习技术员，我叫王小雨。”
“How do you do？ Welcome to our Crew.My name’s Chen Ming, the driller of No.3 Shift.May I help you?”

When the first sun rayed in the next morning, Wang Xiao-yu, who was excited a whole night, got up very early, putting on his new working uniforms and going to the rig-site.He wanted himself to be familiar with the working circumstances as soon as possible.天刚放亮，激动一宿的王小雨早早起床，穿上崭新的工装来到井场。他想尽快地熟悉新的工作环境。
He climbed up the rig floor first.首先，他登上钻台。

Wang Xiao-yu just graduated from the South-west Petroleum Institute, and was assigned to be the practice technician of No.17 Great Wall Drilling Crew.王小雨刚从西南石油学院毕业，分配到长城17钻井队当实习技术员。
It was the first day that he reported to GW17 today.今天是他到长城17队报到的第一天。
He met Mr.Chen Ming, the driller who came from the rig floor, at the well-site.在井场,他遇到了从钻台走下来的司钻陈明。
He went to him and asked: “Excuse me, Sir.My name’s Wang Xiao-yu, a new practice technician of this Crew.”

“Branch cable connection, lamp holder, lampshade of each lamp in the mast is safe and reliable? No problem?”

Asmart and Jahrum Formation is approximately 537m(167-704m)thick and consists primarily of a limestone and dolomite.This sequence is highly fractured and porous.In total, more loss zones in the upper 26” hole section, the lower is stress sloughing shale and tight hole interval, easy pipe sticking.”

The 3rd day of spudding, the Drilling Supervisor, Email was in his office, kept his eyes on real time on-site monitor to observe each changing drilling parameter.Observing almost an hour, he found that the drilling torque increased, rotary speed fluctuated, and the drilling time reduced.He thought it was necessary for him to go to the floor for judging.开钻第三天，办公室里甲方钻井监督Email密切注视着时实监视器上各个钻井参数的变化。在将近1个小时的时间里，钻井扭矩增大、转盘转速有波动，钻时减慢。他想有必要上一趟钻台观察一下。
When climbing up the floor, he saw the Toolpusher holding the brake handle, and George was also observing the change of drilling parameters on the driller’s console.当他爬上钻台，看见带班队长在扶刹把，而George在观察司钻控制台上钻井参数的变化。

“A little, not very obvious,” said the Toolpusher hesitantly, “ I guess maybe some teeth worn out or bit balling”
