船舶和设备的维护程序 Ship and Equipment Maintenance Procedure
1.目的Objective 本程序旨在防止船舶和设备得不到良好或有效的维护而丧失其安全与环境保护功能的要求,也防止公司将不合要求的船舶和设备投入海上营运,确保船舶和设备处于适航与适货状态。
This procedure aims at keeping good and effective maintenance of ships in conformity with requirement of safety and environmental protection, also aims at preventing ships not comply with requirement of seaworthy/ cargoworthiness put into operation.2.适用范围Applicability 本程序适用于船舶和设备的维护保养。
This procedure shall apply to maintenance of ship and equipments.3.职责
Responsibilities 3.1.机务部
Technical Department 3.1.1.机电主管部门,有责任使船舶和设备处于适航状态并满足主管机关、船检机构的要求以及公司方针目标的实现。
Technical Department is responsible for maintenance of ship and complies with requirement of administrations, class society and company regulations.3.1.2.负责制定船舶设备维护周期和分工明细表,并检查、督促和指导船舶制定、实施船舶及设备维护计划;
To be responsible for drawing up the ship‟s annual maintenance plan, to be responsible for supervising of the drawing and carrying out of maintenance of ships and equipments and providing enough guidance.3.1.3.负责船舶备件、物料的审批、供应、管理,审阅、处理船舶上报的各类有关船舶及设备维护方面的技术报告及报表。
To be responsible for approval, supplying, management of ship‟s spare parts and materials;to be responsible for the vetting, filing of the technical reports and relevant records reported from the ship which regarding ship and equipment maintenance.3.2.船舶
Ships 3.2.1.船长和船员是实施维护保养的主体,以保持船舶和设备符合规定的船级要求;
The master and crewmember is mainstay of ship maintenance so as to keep ship and equipment complies with requirement of class.3.2.2.负责船舶和设备维护计划的制定和实施以及确保进行规定的检查;
To be responsible for planning and carrying out ship maintenance plan, ensure inspecting according to regulations.3.2.3.负责船舶物料、备件的使用管理;
Responsible for the using of ship‟s stores, spare parts management.3.2.4.负责向机务部报告船舶和设备的缺陷并向指定人员报告船舶和设备的不符合规定情况。
Responsible for reporting defects of ship and equipments to Technical Department and reporting non-conformities situations to DP.4.工作程序
Working Procedure 4.1.制定预防维护计划 Drawing up of maintenance plan 4.1.1.机务部应根据船舶设备维护周期和分工规定的养护周期,并结合有关规范、规则要求、船检机构的要求、设备说明书、船舶现状及以往设备故障和缺陷的处理情况等,在每年十二月初编写下年度的维护计划,下达船舶执行。
Technical Department should draw up ship‟s Annual Maintenance Plan for next year every December according to maintenance intervals and requirements of relevant international conventions/ Flag State / class / Instructions of Manufacture and the Ship‟s current conditions/dispose situation of former equipment failure and defects etc, and sent to ship for carrying out.4.1.2.该计划应特别规定标识和保障对船舶因突发性运行故障而导致险情的设备和技术系统以及备用设备和长期搁置装置的维护与保养。
The plan should specially provide identification and ensuring of maintenance about equipments have suddenly operation troubles and result in dangerous/technique system/standby equipment/equipments lie idle for long-term.4.2.预防维护计划的实施
4.2.1.船长和船舶各部门长应根据船舶设备维护周期和分工,按年度维护计划中的维护项目每月组织船员开展维护检修工作,并由检修责任人将维护工作做好记录。
The master and the Department Chief should assign the tasks to the respective personnel and organize the ship‟s maintenance as per the maintenance plan and periodic table;the responsible staff should record the maintenance work.4.2.2.在实施过程中,养护人员应遵守船舶轮机设备维护的相关规定。检修责任人必须将负责的维护项目进行认真检查、验收,确保维护检修计划的有效实施。
During the period of implementation the maintenance person must comply with relevant provisions and the responsible person must exercise checking and confirmation for the maintenance items so as to ensure that the maintenance plan be completed with quality and quantity guarantee.4.2.3.船舶各部门长在每月底将养护工作的实施情况分别填写月度维修保养计划表,由船长审核签字后上报机务部主管。
Department chief is responsible for recording implementation situations and filling in < Monthly Maintenance Plan>, master should examine and signed and then submit to technical manager.4.2.4.对本月未能完成的维护项目,部门长应将其记录在各部门月度维护检修工作报表中说明原因,并列入下月维护计划。
Department chief should write the unfinished items in monthly maintenance plan of each department and explain reasons, then list them in plan of next month.4.3.船舶修理 Ship repair 4.3.1.船舶修理类别 Sort of ship repair 分为航修、计划修理(包括检修)和事故修理。Voyage repair, scheduled repair, accident repair 4.3.1.1.航修 Voyage repair 航修属临时性修理,不编计划。主要是为了解决营运中发生的局部故障,影响航行安全,而船舶又不能自修的工程。航修前由船长签署各部门编写的“航修报告”报机务部评审后通知船舶安排修理。航修可由航修厂利用船舶在港期间进行,不影响船舶营运。修理过程中,船上相关人员应按规定做好监修和验收工作。
Voyage repair is temporality repair and not included in repair plan.That aims at settling partial failure which occurred during operation and affect safety of sailing but can‟t be repaired by crewmember.Technical department would review the “voyage repair report” signed by master and inform ship about repair arrangement.Voyage repair could be arranged when ship is in port in order to avoid loss of time.Relevant person should be supervising and inspection the repair work during repairing.4.3.1.2.计划修理(分小修和检修)Scheduled Repair(minor repair and overhaul)4.3.1.2.1.小修:按规定周期有计划地结合“期间检验”或“年度检验”进行的厂修工程。一般每12—18个月为一个周期。
Minor repair: Repair works carried out in shipyard refer to period and combine with “period survey” or “annual survey”.One period lasts for 12 to 18 months.4.3.1.2.2.检修:按规定周期每隔2—3次小修并结合“定期检验”或“特别检验”进行的厂修工程。一般每5年为一个周期。
Overhaul: Repair works carried out in shipyard every 2-3 Minor repair and combine with “periodical survey” or “special survey” 4.3.1.2.3.事故修理:船舶发生事故后,应根据船舶损坏情况和检验部门提出的修理范围和要求进行。如果通过临时修理可以取得适航证书的可做临时性修理,以减少营运损失。如损坏严重则应根据当时当地的条件决定修理方案。事故修理如距计划修理时间较近可以考虑合并进行。重大事故的修理,公司应派机务经理进行监修。
Accident repair: repair work should be carried out according to damages of ship and requirement of relevant survey department once there was one accident.In order to reduce loss of hire temporary repair should be done if that can meet the requirement of seaworthy.Repair plan should be drawn up according to condition of then and there if there is a serious damage.If the accident repair is close to scheduled repair, combining could be considered.A superintendent is need once there was a grave accident repair.4.3.2.修船准备 Prepare work for ship repair 4.3.2.1.编制修理计划 Drawing up repair plan 4.3.2.1.1.船舶修理计划分年度、月度修理计划二种。由机务部经理根据有关规范、规则要求,船检证书和检验类别,设备制造厂的要求,近年来的修理记录,船舶现状及船舶关于设备的失效/损坏、故障和缺陷的反馈信息,在修理年度前的年底编制年度修理计划,报送总经理,审批下达。
There are two kinds of repair plan, annual plan and monthly plan.Manager of Technical Department should draw up ship‟s Annual Maintenance Plan for next year at the end of every year according to maintenance intervals and requirements of relevant international conventions/ ship survey certificates and sort / requirement of Manufacture /records of repair and the Ship‟s current conditions/dispose situation of former equipment failure and defects etc, and submit to general manager of approval, then make known to lower levels.4.3.2.1.2.月度修理计划根据船舶营运安排和年度计划以及船舶实际状况,由机务部经理按年度修理计划中的进厂日期应提前三个月编妥,主要项目为船名、修理类别、计划修期、修船厂名、计划进厂时间,以便于和船舶及船厂联系。
Manager of Technical Department should draw up a monthly repair plan three months earlier then the date of going into shipyard which listed in annual plan according to ship‟s operation, annual plan and ship‟s actual condition.Main items should include vessel name, sort of repair, planed period of repair, name of shipyard, planed time going into shipyard so as to keep good communication with ship and shipyard.4.3.2.2.编制修理项目工程单 Draw up EO-of repair
4.3.2.2.1.船舶各部门长应根据年度修理计划和月度修理计划,在厂修前(2-3)个月编写修理单,由船长审阅后报机务部经理审批。修理单可按坞修、甲板、轮机、电气四个部份编制。
Department chief on board should draw up repair list 2-3 months earlier then shop repair according to annual repair plan and monthly repair plan, then sent to manager of technical manager for approval by master.Repair list could be draw up by four sections: docking, deck, and engine, electric.4.3.2.2.2.机务部收到修理单后,机务经理应上船审查核对,并进行必要的补充和修正,最后修理单的项目由机务部负责定案,船员如有不同意见,应主动介绍情况,说明理由,通过协商,由机务部最后决定。
Once received the repair list, manager of technical department should go onboard for check and examine, make the necessary supplement and correction, the repair list should be finalized by technical department, person onboard should brief and adduce if he has different opinions, then technical department finally decided after discussion.4.3.2.2.3.事故修理由船舶编制修理项目工程单报机务部审核实施。
Refer to accident repair, ship should draw up EO-of repair and submit to technical department for approval and then implement the list.4.3.2.3.进厂准备prepared into shipyard 4.3.2.3.1.船舶进厂前应准备好修船必需备件。重大部件至少应在半年前提出申请,以便及时订货。对于需要的大量材料及特殊材料,在船厂选择定后,应提前通知船厂。
Spares should be prepared before ship enters the shipyard.Big spares should be applied for at least 6 months advanced so as to order timely.If mass materials or special materials are needed, an advanced notice should be given to the selected shipyard.4.3.2.3.2.为了便于船厂做好修船准备,应尽可能安排船厂人员上船看工程,以便争取船期,船舶进厂前,船长应组织各部门长,统一指挥修船工作,研究处理修船中的技术业务问题。
It would be better to arrange the worker of shipyard go on board to check the repair project so as to save repair time.Master should organize every department chief, command the repair work in unify and deal with technical problem during repair.4.3.2.3.3.进厂前应备妥施工可能使用的图纸资料、专用工具、清理影响施工的物品、危险品,为施工提供安全工作条件。
Information and drawing, special tools should be prepared before shop repair, obstacle and object would affect construction should be cleared and provide safe working conditions.4.3.2.3.4.船舶修理范围涉及主机、锚机、舵机等工程,如厂方要求可在修理前进行预试航。预试航由船舶、机务、修方各派员参加。
Ship repair project includes main engine, windlass, and steering engine;trial trip could be carried out if required by shipyard.Ships, technical department, shipyard should design person to attend the trial trip.4.3.2.4.增减工程 Increase and decrease of project 4.3.2.4.1.船舶进厂后,根据检查的情况允许对工程范围内的项目进行修正。但增减工程项目范围必须严格控制,在修船过程中,允许在修理合同三分之一的修期内增减项目,以免影响工程进度。
Items of repair project could be revised refer to result of checking when vessel in shipyard.The increase or decrease range should be strictly controlled, In order to avoid impact progress of works, only changes before former one-third of whole time is permitted when vessel is under repair.4.3.2.4.2.增减工程项目应由船长与机务经理决定。
Increase or decrease of project should be decided by master and manager of technical manager.4.3.2.5.监修和验收
Supervision and accepting
船舶进厂修理时,公司应派出机务经理或主管,负责与船厂联系,决定修理工程,处理修船中发生的问题。船舶应负责具体修理项目的监修和验收工作。
The company should appoint manager of technical department or person in charge to be responsible for keeping communication with shipyard, decide repair project and deal with problems during the repair.Ship to be responsible for supervising, examining and receipting the repair work.4.3.2.5.1.监修
Supervising 4.3.2.5.1.1.一般工程由轮机长和大副分别组织人员监修,重大工程应由轮机长和大副亲自监修。
Chief engineer or chief officer should supervise grave projects by themselves and appoint person to supervise general projects;4.3.2.5.1.2.监修人负责监督船厂是否按修理单指定的范围和要求施工;
Superintendent is responsible for supervise whether the shipyard construct according to requirement of repair list.4.3.2.5.1.3.工艺、材料及安装质量是否符合技术要求;
Whether technology, quality of material and installing are pass muster.4.3.2.5.1.4.施工中有无船厂责任引起的部件及设备的损坏;
Whether any damage of spare or equipment caused by responsibility of shipyard.4.3.2.5.1.5.施工中有无不安全因素,可能引起火灾及其它危险,必要时有权停止其施工,并向主管人员汇报,等待处理。
Is there any factor of unsafely which may cause a fire or other dangerous, superintendent is authorized to stop construction as necessary, and then report to people in charge for settling.4.3.2.5.1.6.做好必要的修理记录,以便对验收和审核帐单准备材料;
Make necessary repairs records so as to get prepared for accepting and checking debit note accordingly.4.3.2.5.1.7.监修人应配合船厂工作,为施工提供方便条件。
Superintendent should coordinate with the shipyard and provide convenience for repair project.4.3.2.5.2.验收
Acceptance 4.3.2.5.2.1.施工过程中,船员应切实了解工程进度和做好质量验收工作。船厂施工完毕应交船员验收,验收时应由船、厂双方代表在场,验收后由验收人签字,作为该项工程的结束。所有船检要求检验的项目,必须督促厂方按期申请交验。除船检规定要求检验项目外,为确保修船质量,船方的质量验收应做到:
Crewmember should follow up the progress of works and get prepared for acceptance.The shipyard should ask crewmember to examine and accept the project with representatives of both ship and shipyard be present when they finish repairing work, then person in charge of accept signed for acceptance and then the work over.Refer to items required by ship inspection department should be finished on time.Besides items required by ship inspect department, in order to ensure qualify of repair work, qualify acceptance should include:
4.3.2.5.2.2.一般修理项目,需船方部门负责人检查满意
General repair item should be examined and accepted by department chief.4.3.2.5.2.3.水线以下工程的施工安装、验收,大副、轮机长必须亲临现场,必要时机务经理或主管参加验收。
Project below waterline should be supervised and accepted by chief officer and/or chief engineer, manager of technical department or person in charge should attend for acceptance if necessary.4.3.2.5.2.4.对影响航行的重要机电设备,如主机、发电机、舵机、锚机等的修理,从拆下、解体、加工、安装、测试、试车等过程,船舶主管人员必须到现场共同检查,机务经理或主管须随时到现场掌握修理质量的动态。
Refer to repair of those important electromechanical equipments which affect safety of sailing, such as main engine, generator, steering engine and windlass etc, governors onboard should examine the whole process include demounting, disassembly, machining, installing, testing, and trial run, technical manager or person in charge should attend the sense and master process/qualify at any time.4.3.2.5.2.5.厂方必须准确、及时提交修理船舶质量检验书面报告,一份交机务部,一份交船。
The shipyard should submit exact Repair Quality Inspection Report to Technical Department and ship.4.3.2.5.2.6.单项工程完毕后,可进行单项验收。需要运转试验的设备,应进行运转试验。必须试航才能验收的项目,可留待试航时验收。凡不符合要求的项目,应由船厂负责解决,双方如有争议,首先进行协商,如协商不成,以船检部门的要求为准。
Single project could be examined and accepted separately once finished.Equipment should be tested run if needed.Items needed to be examined during trial trip should be waiting for trialing trip.If any undesirable item, the shipyard should be responsible for rectification, any dispute should be consulted firstly and if fails, requirement of ship inspection society should be applicable.4.3.2.5.2.7.修理完工后可根据修理范围决定是否需要试航,或在码头试车。试航时应由双方提出试航大纲,明确试航时的安全责任。试航中发现的问题,凡应厂方负责的项目应由船厂负责修理。
Whether a trial trip or test run is needed should be refer to extent of scope of repair.Compendium of trial trip should be done by both two sides and clear safety responsibility in trial trip.Problems found in trial trip should be settled by shipyard in their responsibility range.4.3.2.5.2.8.修理完毕,应组织力量,认真审核完工单。完工单是编制帐单的主要依据,要严格把关,属于质量未达到要求的应文字注明,并双方签字。
The Completion List should be checked carefully and strictly when repair is finished for that‟s main basis of preparing the debit note.Unqualified repair work should be marked out by text notation and signed by both side.4.3.2.6.船舶自修
Self-repair onboard 船舶自修分为营运期间的自修和厂修期间的自修两种。
Self-repair includes repair during operation and repair in shipyard.4.3.2.6.1.船舶营运期间,船员在完成预防检修计划的前提下,可以自修属于厂修范围的工程,一般不占用营运时间,但对运输任务繁忙、停泊时间短,或船舶定员偏少的船舶,可由公司给予一定的停航检修时间,必要时可申请船厂、航修厂协助。
When ship is under service, crew member could do some repair works in premise that they finished the prophylactic repair plan, which always don‟t occupy time of operation.But to that ship which is under busy service, short anchor time or have less person onboard, the company should give some time for repair suspend service, help of shipyard should be arranged if necessary.4.3.2.6.2.船舶厂修期间,船员应尽可能多地做一些属于厂修范围的自修工程,进厂前应在编制修船计划的同时编制自修计划,由机务部批准后执行。自修计划应与厂修相配合,按厂修预计时间,除监修及日常工作外的人员,能力统筹考虑。
Crew members should do some self-repair works as more as possible when vessel is under repairing in shipyard.A self-repair plan should be drawn up together with repair plan before ship enters shipyard and implemented when approved by Technical Department.Self-repair plan should comply with time of shop repair, and make competitive arrangement about labor power except superintendent and person in daily duty.4.3.2.6.3.自修、厂修工程应注意不要互相干扰,自修工程应与厂修同时完工,不影响开航。Self-repair and shop repair shouldn‟t in mutual interference, and should be finished at the same time so as to set sail on time.4.3.2.6.4.自修工程应严格控制质量,工程完毕后由船舶领导负责验收。对重大的自修项目,应请机务经理或主管亲自验收。
The quality of self-repair should be strictly controlled, and leader onboard should check and accept the repair work themselves.Refer to important items must be accepted by technical manager or person in charge.4.3.2.7.安全保卫
Security 4.3.2.7.1.进厂后船舶领导应及时与船厂联系,了解船厂的安全保卫制度,向船厂说明有关船舶安全保卫的要求,共同落实防火、防风、防污染等各项安全措施,并签订“安全保卫协议”。
Leader onboard should contact with shipyard timely once ship entered shipyard, inquiry security system of shipyard and state security requirement of the ship, carry out each safety measure jointly such as fire prevention, anti-pollution etc, and sign „Agreement of Security‟.4.3.2.7.2.在厂期间的抗灾施救工作应服从船厂的统一指挥,但应积极做好各项安全准备工作,一旦发生灾害性事故,全船应坚守岗位,奋力抢救,保卫船舶和人员的安全。
Disaster rescue should follows shipyard‟s unified command when vessel in shipyard, but the ship should get prepared for security, once there is a disaster accident, the whole ship should hold position, struggle for the rescue, and defend safety of vessel and crew.4.3.2.8.修船总结
summary of repair 4.3.2.8.1.修船结束后,船长应及时组织各部门人员进行总结,在一个月内向机务部写出书面修船总结报告。总结报告包括的内容:
Master should organize every department for summarizing and submit a writer report to Technical Department in one month form date of finishing.The report should include: 4.3.2.8.2.修船地点、时间、修理厂、修理类别; Place/time/shipyard/sort of repair 4.3.2.8.3.修理主要项目,修理质量,对船厂工作的评价及自修工作情况;
Main repair items, quality of repair, evaluation about work of shipyard and status of self-repair.4.3.2.8.4.修船遗留的问题及以后修理注意的事项和建议; Problem left over, items should be noticed in future and advices.4.3.2.8.5.附修理的检查测量记录及其它技术档案资料,以便机务部审阅后将其作为船舶单船技术档案归档。
Check measurement record and other relevant technical materials should be submit to Technical Department for check and approve and then filed.4.3.2.9.修船质量反馈 Repair quality feedback
4.3.2.9.1.船舶修理项目在质量保证期(固定件六个月、运动件三个月)内发生质量问题,船长应及时报告机务部。
If the repaired item has C.Quality Issues in warranty period(six months for fixed block, three months for moving part), master should report Technical Department immediately.4.3.2.9.2.机务部应与承修船厂或航修厂进行交涉,综合分析、处理。
Technical Department should negotiate with the shipyard, analysis by synthesis and deal with the problem.4.4.物料、备件
Ship stores, Spare parts 船舶检修和修理所需备件与物料的供应,按《船舶备件、物料管理须知》执行。
Supplying of ship stores and spare parts for overhaul and repair should be implemented according to < Ship spare parts & material management Instruction>.4.5.船舶和设备的检查 Check of ship and equipment 4.5.1.船上检查
Check on board 4.5.1.1.船长应检查、督促船舶各部门按检修计划对船舶和设备实施有效的维护。Master should exercise checking and supervising every department onboard implementing of repair schedule on board so as to ensure the ship and equipments maintained effectively.4.5.1.2.船舶开航前检查按相关的规定执行。
Examination should be carried out according to relevant previsions.4.5.1.3.船舶安全自查按《船舶安全检查制度》的规定执行。
Self inspection of ship safety should be implemented according to < Ship Safety Inspection System>.4.5.2.公司检查 Company inspection 公司相关职能部门对船舶的检查按《船舶安全检查制度》的规定执行。
Inspection of company‟s relevant function department should refer to < Ship Safety Inspection System >.4.6.报告和实施纠正措施 Reporting and implementing corrective action 4.6.1.船舶在维护和在检查中发现的设备故障或缺陷,如船上有能力自行修理解决,应迅速予以纠正,并将纠正情况记录在船舶和设备维护报告表报机务部。
Refer to equipment failure and defect found in maintenance and check, corrective action should be carried out immediately if that can be repaired by crew member, and make relevant records in the Ship/Equipment Maintenance Report and report the same to the Technical Department.船上纠正能力以外的设备缺陷及故障,船长应及时填写表格《船舶设备修理申
请表>报机务部。在航或外港的船舶可采用其它方式(电子邮件、传真、电话、等)上报。报告的主要内容为:日期、船位、设备名称、情况简述(包括可能的简图及原因)、已采取的措施、需要的备件或措施等。机务经理或主管应及时对报告进行评审和向船舶反馈处理意见。
For those deficiencies /breakdown of which can not be rectified by crew member, the master should write < Ship‟s Equipment Repair Requisition> and submit it to Technical Department.Ships underway or in foreign port could report through other way(email,fax, telephone, etc).Report should include: date, ship's position, name of equipment, Description of situation(Including possible diagram and reasons), actions taken, spare parts or measure needed.The Technical Department should make verification for the deficiencies reported and feedback disposal opinion to ship.4.6.2.如机务部认为发现的故障及缺陷影响船舶安全航行和环境保护,按船舶维护程序的有关规定立即安排修理并通知船方,机务经理或主管须对船舶和设备的修复情况进行信息跟踪,以确保船舶安全。
If the Technical Department thinks the breakdown or defect would affect safety of ship and environment, they should arrange a repair immediately according to relevant provisions and inform the ship, technical manager of governor should follow the process of repair so as to ensure safety of ship.4.6.3.如机务部认为发现的故障及缺陷不影响船舶安全航行和环境保护,机务经理或主管应将处理意见反馈给船方,并做好登记,在下年度修理时予以解决。
If the Technical Department think the breakdown or defect won‟t affect safety of ship and environment, technical manager or governor should feedback disposal opinion to ship and make record, then repair it in next annual repair.4.6.4.如果是人员管理、操作不当或SMS文件的缺陷引起的船舶和设备导致的缺陷或故障,应按《不符合规定情况、事故和险情的报告和分析、纠正程序》中的规定执行。
If the equipment failure or defect was caused by personnel management, misoperation or defect of SMS files, that should be settled according to < ReportingAnalysis and Rectification Procedure for Non-Conformities, Accidents and Miss Risk Occurrences>.5.记录 Records 5.1.船舶设备的预防维护周期检查维护清单

—月度预防检修计划。责任 4.1 船长
对船舶和设备预防检修计划的执行情况负责。每月要对各部门上报的设备养护执行记录签字确认。负责向公司报告船舶及设备上的缺陷,并落实公司的指令,负责落实缺陷的纠正。4.2 船舶部门负责人(大副、轮机长)
大副、轮机长分别负责编制本部门的-月度预防检修计划。根据检修计划编写部门月度计划,负责执行计划,每月向船舶机务主管报送上月“月度计划执行情况表”。并及时上报有关设备的检查、测量的数据记录和报告表。负责监督和指导设备检修负责人实施设备的日常维护保养工作。负责把本部门的设备维护、保养情况及设备的技术状况向机务主管汇报。负责检查船体和船舶设备状态。船舶修理时在机务主管指导、安排下与承修厂协作解决本船重大或疑难问题。负责对厂修、自修工程质量的监督和验收。发现设备的缺陷和管理的不符合后,负责按P0901《不符合、缺陷报告和处理程序》规定执行,并负责纠正、预防措施的实施和监控。
4.3 设备主管人员
负责主管设备的维护保养,保证设备处于良好的工作状态。领导、指导养护人员对主管设备按规定正确保养、维修。对所管设备的日常维修、检修质量负责检查和验收。所有维修保养情况要记录在《检修工作记录簿》中。记录各项测量数据和报表,并按规定送部门长审核上报。负责主管的船舶及设备缺陷分析原因,并向部门长报告,按照公司指令落实纠正措施。4.4 各船舶的机务主管
审核船舶甲板、轮机预防检修计划及每月执行情况; 负责落实修理安排、检查修理安全和质量,管理船检证书副本。在船舶及设备维护方面,给船舶以足够的支持。对船舶及设备维护情况进行定期检查。相关的检查可分解到海务、通导及电气主管,如发现缺陷时,按P0901《不符合、缺陷报告和处理程序》规定执行,找出原因,采取纠正措施,记录并保存。检查船舶对于关键性设备和技术系统定期检查和试验的实施情况。编制单船修理计划报部门经理。4.5 公司机务部
制订公司-月度维修保养管理规章制度。根据各机务主管上报的修理计划,审核并编制船舶修理计划。5 实施步骤
5.1 船舶-月度预防检修计划执行
5.1.1 机务主管根据船舶和设备情况,考虑适用的国际公约、船旗国和港口国强制性规则、船级社规范、制造厂商的要求和说明,及公司长期以来的经验积累,制订出《船舶甲板部预防检修计划表》、《船舶轮机部预防检修计划表》,对预防检修计划表中的项目确定常规保养周期。
5.1.2 船舶要根据船舶设备的实际情况及设备说明书规定的养护要求,编制《甲板部预防检修计划表》、《轮机部预防检修计划表》并作好月度预防检修计划;凡没有预先编定的保养周期,可根据设备说明书或厂方提供的保养手册要求船上自己确定养护周期,编制时要充分考虑接受船级社年检和特检项目的时间安排;执行“循环检验”的船舶还必须将检验项目做合理的安排。此外凡有计划厂修和坞修的也要对修理项目的维修做全盘考虑做好安排。船舶编制完成预防检修计划表于每年11月前将计划上报公司机务部。
5.1.3 大副、轮机长每月根据批准的《甲板部预防检修计划表》、《轮机部预防检修计划表》,将当月的维修保养项目,填入《月度预防检修计划和执行记录表》中。并将每月的维修保养项目分解到设备主管人员;要注意必须明确指出维修保养的内容、要求及试验验收的规定。
5.1.4 考虑到船舶运行的特殊情况,当月不能按计划完成的维修保养项目要尽量安排在下月完成,如果长期不能完成的,要报机务主管安排适当时间完成,机务主管要做好备案。
5.1.5 设备主管人员对设备进行养护后,要在《轮机日志》、《电气日志》、《GMDSS电台工作日志》、《检修工作记录簿》、《消防、救生设备登记检查养护记录簿》、《蓄电池维护或更换记录》等上做好记录,并且要报告部门长设备维护工作完成的简要情况,部门长在《月度预防检修计划和执行记录表》上记录。
5.1.6 设备检修后各种测量记录要按规定填写,由部门长签字认可。
5.1.7 每月底大副、轮机长和电机员都必须将《月度预防检修计划和执行记录表》、《船舶应急设备检查执行情况表》向船长报告,经船长签字后一式两份,一份留存,一份报送机务主管。
5.1.8 机务主管对船舶的执行情况进行审阅。如发现问题,应分析其原因,提供物质支持或技术指导。
5.1.9 船舶通信、导航设备的维护参照I 1001-07《船舶通信导航设备维护保养规定》,船舶通信、导航设备的检查参照I 1002-02《关键性设备及技术系统定期检查和测试规定》。6 船舶及设备缺陷情况的处理
船员在日常的船舶及设备维修保养(包括PSC/FSC检查、离、抵港前检查等)中,发现船舶及设备的缺陷应按P 0901《不符合、缺陷报告和处理程序》要求进行处理。相关须知文件

4.0设备设施维护保养程序
1.0目的对公司设备设施进行维护保养,为正常生产提供和维护所需要的设施,确
保产品质量符合要求。
2.0范围
a)厂区内环境设施保养
b)厂房和场地维修保养
c)工器具维修保养
d)仪器设备设施维修保养
3.0职责
a)生产车间、品管部每年12月制定下的《设备设施维护计划》,由
总经理审核批准。
b)生产车间负责对保证正常生产和确保产品质量符合要求所需的设施进
行跟进。
c)品管部负责对生产工作环境进行控制。
4.0程序
4.1生产设施的定义:公司为保证正常生产和确保产品质量符合要求所需的设施,包括:工作场所(车间、办公场所等)、设备和工器具(包括生产设备、工具、器具、卡具、量具)、软件(计算机网络)、公用设施(水、电、汽供应)、通讯设施、运输设施等。
4.2生产设施的提供
4.2.1生产车间、品管部门根据使用的要求及公司发展的需要,说明设施名称、型号
规格、用途、技术参数、单价、数量等,经总经理审核,总经理批准后,由生产车间、品管部门负责组织安排采购或自制的有关事宜,采购部门具体实施采购;
4.2.2需要自制的设施由使用部门提出,由生产品管部设计,经总经理审核,总经理
批准后,由生产品管部门要求加工制造。
4.3生产设施的验收
4.3.1采购或自制完成的设施,生产车间、品管部组织使用部门进行安装调试,确认
满足要求后,由生产品管部门和使用部门在验收单上签字验收,并记录设施名称、型号规格、技术参数、单价、数量、随机附件及资料等内容。设施验收单由生产品管部保管。低价值易耗品,如:工具、卡具、量具等由使用部门自行验收;
4.3.2验收不合格的设施,生产车间、品管部与供货商协商解决,并在设施验收单上
记录处理结果;
4.3.3生产车间、品管部对验收合格的设施进行编号,建立《设备标识卡》和设施档
案,并在《生产设备一览表》上登记;
4.3.4生产车间、品管部根据设施验收单办理入厂登记和建档手续;低值易耗品由仓
管人员凭设施验收单办理入库手续。
4.4生产设施的使用、维护和保养
4.4.1根据生产需要,生产车间、品管部组织编写设施的操作规程,发放给使用部门。
对于大型的精密设备或关键、特殊过程所用的设备必须有操作规程,相关的操作人员应由品管部门负责培训,考核合格后,持证上岗。
4.4.2生产车间、品管部共同制定《设备维护保养计划》,规定维护保养项目、频次,交给使用部门执行,各岗位负责人监督检查执行情况。生产车间、品管部每季度收集《设备维修记录表》,整理入档并作为制定检修计划的依据。
4.4.3生产车间、品管部每年12月制定下的《设备检修计划》,由总经理审核批
准后发给各部门执行。
4.4.4日常生产中车间无法排除的故障,应报生产品管部门检修。检修中的设施应挂
红色检修牌,检修好的设施应有使用部门负责人签字验收方可使用。生产品管部门应将检修发现的问题记录在相应的《设备标识卡》上。
4.4.5现场使用的设施应有编号标识,以便维护保养。
4.5生产设施的报废
4.5.1对无法修复或无使用价值的生产设施,由生产品管部门填写《设备报废单》,经
总经理批准后作报废处理,生产车间、品管部在《设备标识卡》及《生产设施一览表》中注明报废情况。
4.5.2对工具、卡具、量具等低值易耗品,由使用部门填写《设施报废单》,报生产车
间、品管部领导批准后,即可报废。
4.5.3等待报废处理的设施应挂报废牌。
4.6工作环境
4.6.1财务部应协助生产车间、品管部识别并管理为保证正常生产和确保产品质量符
合要求所需要的工作环境中人和物的因素,据生产作业需要,负责确定并提供作业场所必须的基础设施,创造良好的工作环境,包括:配置与生产加工能力相适宜的车间厂房,并根据生产需要适当装修,以利于出口食品生产加工;配置必要的通风、空气调节和消防器材,保持适宜的温度、湿度,以利于安全卫生生产; 生产车间、品管部对车间设施的配置和布局实行动态管理,要考虑人体工效学的要求,努力提高工作效率;
确保员工生产符合劳动法规的要求。

船舶过户程序和手续
先由旧船主办理船舶营运证注销手续。新船主持港航部门出具的《中华人民共和国船舶营业运输证注销登记证明书》到港航分局领取《船舶买卖、检验入(过)户呈批表》,加具单位意见并盖章(个体到村委或居委会加具意见并盖章);持已盖章的《船舶买卖、检验入(过)户呈批表》到海事部门办理船牌证件转让(过户)手续;填写在港航分局领取的 《国内水路运输登记事项申请表》、《安全管理责任书》,加具单位意见并盖章(个体到村委或居委会加具意见并盖章),个体还需由村委或居委会出具船舶买卖证明;持以上资料及新的船舶检验证书、船舶国籍证书、所有权登记证书等证件到港航分局进行初审,再由市港航局核发新的船舶营运证和水路运输许可证

船舶抵押程序
一、船舶基本情况调查
确定抵押船舶的具体情况,包括船名、船舶的识别号、船舶的登记号、船舶呼号、船籍港、船舶的种类、尺度、吨位等
二、船舶所有权情况调查
调查船舶所有人/抵押人的地址、联系方式等,确认船舶是否为其所有,取得船舶所有权登记证书,或船舶建造合同及所有权归属证明文书(在建船舶);调查船舶是否存在已抵押情况。
三、船舶抵押确认
取得承租人对船舶设立抵押的确认文件:船舶由多人共有的,取得2/3以上份额或约定份额的共有人同意设定抵押的证明文书,船舶已办理光船租赁登记的,取得承租人同意船舶设定抵押的文书。
四、船舶价值评估
资产评估机构进行船舶价值评估,出具船舶价值评估报告,双方签订对船舶价值认可的确认书。
五、签订抵押合同
确定抵押合同中抵押担保的债务的数额、抵押金额、抵押担保债务的偿还期限、抵押债务的利率。船舶原已设立抵押登记的,进行抵押权转移的,取得抵押权转移合同、抵押人通知抵押权人证明文书、原抵押权登记证。
六、进行船舶抵押登记
船舶抵押应到船舶登记机关办理船舶抵押登记,未经登记的,不得对抗第三人。船舶抵押登记时应提供抵押双方的身份证,委托第三人办理抵押登记的,还应提供委托书及受委托人身份证。除此之外,应填写抵押权登记申请书。申请书的格式如附件所示。
七、抵押权的实现
船舶抵押权人实现船舶抵押权的方式有“折价”、“拍卖”和“变价”三种。在物权法第一百九十五条规定了抵押权实现的条件、方式和程序:债务人不履行到期债务或者发生当事人约定的实现抵押权的情形,抵押权人可以与抵押人协议以抵押财产折价或者以拍卖、变卖该抵押财产所得的价款优先受偿。抵押权人与抵押人未就抵押权实现方式达成协议的,抵押权人可以请求人民法院拍卖、变卖抵押财产。虽有上述规定,但实践中鲜有抵押权人与抵押人就抵押权的实现方式达成协议。因缺乏配套的程序性规定,抵押权人也未能直接向法院申请拍卖、变卖财产,抵押权仍通过诉讼予以实现。审判实践中,抵押权的实现通常是经过诉讼程序,取得生效判决以作为执行根据,再拍卖抵押船舶实现抵押权。
在具体实现程序上,可参照厦门海事法院裁定中信银行厦门分行申请实现船舶抵押权案——根据民事诉讼法实现担保物权的规定适用非讼程序,船籍港及船舶所在地海事法院对此享有管辖权,并有权作出可以作为执行名义的终审裁定。
