中国东盟自由贸易区优惠原产地证明书填制说明

【产地证标题栏】(右上角 Reference No.之后)：此栏应填写证书号。

证书号编号方法如下：证书类型+年份+企业产地证注册号+ 证书序号（共16位）

例：产地证注册号为“370900001”的企业2009年第1份证书的编号为：E09370900001000

1【第l栏】（出口商的名称、地址和国别）：应填写中国境内出口商的名称和地址。

注意：此栏是带有强制性的，必须填写中国境内的出口商详细名称、地址和中国国名，不能填写中国以外的出口商。

【第2栏】（进口商的名称、地址和国别）：一般应填写给惠国最终收货人名称、地址和国别(即信用证上规定的提单通知人或特别声明的收货人)，但不要填中间转口商的名称。

注意：

1、此栏请勿留空，如不体现具体进口商，应在此栏填写“TO ORDER”；

2、如果体现具体进口商，必须填写进口国的全名。

【第3栏】（运输方式及路线）：应在相应的提示后依次填写货物的出运日期、运输工具名及目的港。

注意：

1、货物出运日期必须与提单日期一致；

2、运输工具名必须与提单所列运输工具名称一致；

3、目的地必须是给惠国境内的港口或城镇。

4、凡经香港或澳门转运至东盟各国的优惠贸易协定项下的出口货物，在获得检验检疫机构签发的《中国-东盟自由贸易区优惠原产地证明书》后，申请人需持该证书及有关单证，向香港中国检验有限公司或澳门中国检验有限公司申请办理“未再加工证明”。

【第4栏】（供官方使用）：此栏由给惠国海关填写，出口企业应将此栏留空。

【第5栏】（项目序号）：一般情况下显示为“1”，在收货人、运输条件相同的情况下，如同批出口货物有不同品种，可按不同品种或不同发票号等分列“1”、“2”、“3”…。

【第6栏】（运输唛头及包装号）：按发票上唛头填写完整的图案文字标记及包装号。

例如：NO．1-160

注意：

1、如货物无唛头，应填“N／M”或“NO MARK”等；

2、如唛头过长此栏不够，可打在第7栏结束线下以及第8、9、10栏的空白处；

3、如唛头特别长或者图案标记特别复杂，应该在此栏填写“SEE ATTACHMENT”同时将

唛头打在专用的唛头附页上，唛头附页应该打上该证书号和该证书的签证地点和签证日期。

【第7栏】（品名和包装数量及种类）：应填商品的具体名称和HS编码、包装种类及数量。

例如：ONE HUNDRED（100）CTNS OF APPLES

注意：

1、商品的名称必须具体，限于东盟成员国降税清单内产品。

2、包装种类必须填写具体运输包装，如CTN、DRUM、PALLET、CASE等。但裸装及散装

商品应注明“IN BULK”，挂装服装应注明“HANGING GARMENTS”。

3、包装数量必须在注明英文大写的同时注明相应的阿拉伯数字。

4、国外信用证如要求填写合同号、信用证号、提单号等说明内容时，可填写在此栏结

束符下方的空白处。

5、进口国HS编码即为我国现采用的国际上协调统一的HS编码，要求填写六位数或八

位数的HS编号。

6、在正文下方必须加打结束符。

【第8栏】（原产地标准）：此栏是普惠制原产地证书的核心内容。一般填写规定如下：

1、对完全原产于中国，不含进口成份的货物，此栏应填“X”；

2、对非完全原产于中国，但在该国的生产符合中国-东盟自由贸易区原产地规则中规则2所列的原产地标准的产品，被另一成员国用于其最终产品中，只要此最终产品的中国-东盟自由贸易区成份累计不低于40%，则此栏应填“东盟国家原产成份累计的百分比”，例如40%；

3、对非完全原产于中国，为履行中国-东盟原产地规则中规则2（b）的条款，在产品的生产加工过程中，所使用的非成员国或不明原产地的原料、部件的价值不超过所加工获得的产品离岸价的60%，且最后加工工序是在中国境内完成的产品，则此栏应“单一国家原产成份的百分比”，例如40%；

4、满足特定产品规则条件的产品，此栏应填“特定产品规则”。

【第9栏】（毛重或其他数量）：一般应填写毛重，填写净重时应注明“N.W.”或“NET WEIGHT”。以其它计量单位计量的，也可填写其它计量单位。同时填写出口货物的FOB总值（以美元计，且与发票所列FOB值相符）。如FOB QINGDAO USD2000。

【第10栏】（发票号及日期）：此栏不得留空，应按正式出口商业发票填写号码和日期。例如： WF0100

注意： 为避免月份、日期的误解，月份一律用英文全称或通用缩写法如JAN.FEB等表示。发票内容，必须与证书所列内容和货物完全相符。

【第11栏】（出口商的申明）：应填出口商申请的地点、日期以及进口国国名。

注意：

1、申请单位的法人代表或其授权的手签人员必须在此栏中签字并加盖申请单位在签证机构注册备案的产地证中英文印章。非经授权备案的手签人员本人签字和非经授权备案的印章无效；

2、此栏申请日期不得早于第10栏发票日期，也不得同于或者迟于第12栏签证日期，但一般不要迟于提单日期。

3、为避免月份、日期的误解，月份一律用英文全称或通用缩写如JAN.等表示。

【第12栏】（签证当局的证明）：应填签署地点、日期。

注意：

1、签证印章加盖在证书第12栏，证书正本和3个副本均需加盖印章；

2、此栏日期不得早于或者同于第11栏签证日期；

3、为避免月份、日期的误解，月份一律用英文全称或通用缩写如JAN.等表示。

4、进口给惠国的国名必须是正式的全称或缩写。

5、后发证书应由检验检疫机构在第12栏填写“ISSUED RETROACTIVELY”

1、如果已签发的证书正本遗失或损毁，申请单位可在产品出运后1年的期限内，持原证书第3副

本向原签证机构申请重发，经检验检疫机构审查，同意重发时，应由检验检疫机构在第12栏加注：“CERTIFIED TRUE COPY”。签证时间应为原证书的签证时间。

背页填制说明

证书编号：授权签证机构签发原产地证书的序列号。第1栏：填写受惠国出口商的名称、地址（包括国家）。

第2栏：在知道实际生产商的情况下，填写受惠国生产商的名称、地址（包括国家）。如果出口货物由一家以上生产商生产，仅填写最后实际性加工生产的生产商的名称、地址（包括国家）。如果生产商和出口商相同，应填写“同上”。如果不知道生产商，可填写“不知道”。第3栏：填写中国关境收货人的名称、地址。

第4栏：填写运输方式及路线、离港日期、运输工具编号、装货港口和卸货港口。第5栏：可以填写顾客订货单号码，信用证号码等其他信息。第6栏：填写货物项目号。第7栏：填写唛头及包装号。

第8栏：填写货品名称、包装数量及种类。如果是散装货，应注明“散装”。在商品描述末尾加上“***”（三颗星）或“”（结束斜线符号）。第9栏：填写货物对应的《协调制度》六位数编码。

第10栏：若货物符合原产地规则，出口商必须按照下表所示方式，在本证书第10栏中申明其

背页说明

第一栏：应填写中国出口商的法人全称、地址（包括国家）。

第二栏：应填写新加坡收货人的法人全称、地址（包括国家）。

第三栏：应填写运输方式、路线，并详细说明离港日期、运输工具的编号及卸货口岸。第四栏：不论是否给予优惠关税待遇，进口方海关必须在相应栏目标注(√)。第五栏：应填写项目号码。

第六栏：应填写唛头及包装号码。

第七栏：应详细列明包装数量及种类。详细列明每种货物的商品描述，以便于海关关员查验 时识别。商品描述应与发票所述及《协调制度》的商品描述相符。如果是散装货，应注明“散装”。在商品描述末尾加上“***”（三颗星）或“＼”（斜杠结束号）。第七栏的每种货物应填写《协调制度》六位数编码。

第八栏：若货物符合原产地规则，出口商必须按照下列表格中规定的格式，在第八栏中标明

第十栏：对应第九栏中的货物名称注明相应的协调制度编码，以六位编码为准。
