要求：

1.上述第三、四项要求用WORLD文档形式书写，并提交电子版。

2.上述所有信息存放在一个文件夹中，以学生姓名、学号命名后，以指导教师为单位上交电子版。

3.以上信息在5月26日前由各专业（教研室）干事统一上交至迟俭辉老师处。
