女,48岁,初中文化,已婚,退休工人。失眠,情绪低落2个月,伴有想死念头3天,由丈夫和弟弟来陪。
我是退休半年了,刚开始还好。后来就觉得无聊,很烦躁。想打电话给丈夫又怕影响他工作,儿子在读大学也很忙。晚上睡不着,有时只睡1—2个小时,心情也越来越差,做事没精力,对什么事都没兴趣。每天早上丈夫出门的时候,就开始担心这一天该怎么过,他下班后会好些。记性也越来越差,连做饭都会忘放调料。我越想越觉得自己没用,再这样下去会害了丈夫和儿子的,所以,我就想死了算了(边说边哭泣)。
丈夫反应:我妻子初中毕业后就进工厂了,一向工作兢兢业业,是劳动模范。半年前退休后在家,刚开始很好的,很勤快的作家务。但2个月前,我发现她好像有心事,整天闷闷不乐的,做事没有精神,说话反应慢。晚上睡觉时老翻来覆去睡不好,我看她精神不好就劝她去看看医生,大概一个半月前我带她去看内科,医生说她是“更年期综合症,失眠”,配了些安定,吃了药后睡眠好了些,看上去精神也好了些。

自我陈述:因反复做事,犹豫不决,自知不必想或不必做的事仍要想、要做,为之痛苦紧张3年。求助者14岁开始遗精,由偶然渐至频繁,多达每日数次,有时手淫。对此事不敢声张,暗中自责。15岁考入高中,因学习日渐紧张,对担任学生干部社会活动多,影响学习有顾虑,老 1 师提出优秀学生干部考大学时可降低一档(4分)录取分数线,求助者常为此十分矛盾,既怕耽误前程,又怕辜负老师,冥思苦想,难以决断,辗转不眠。
每每下定决心,但又次次动摇,很难完成一件事情,无休无止,脑袋发胀,心情烦躁,全身倦怠,学习成绩下降。自感问题严重,于是强迫自己学习,制定学习方案,但经常是手捧书本,思想却云游四方。自称是“该记的记不住,该忘的忘不掉”。最近半年很担心自己的身体健康,心情烦闷,感到自己不如别人,做事更加小心与缓慢。做事反复多次,同一个问题总要去想。为此而来咨询。

孙某,男性,28岁,未婚,也一直未交女朋友,现就读某著名高校硕士研究生三年级。自我陈述:
担心,紧张,烦躁,1个月以前开始怀疑自己生病,自己求医就诊。
由于一直就读工科院校,学校男女生比例严重失调,所以始终未找到女朋友。1个半月前,同在这个城市中学未毕业就开始做生意的老同学,带着我说是“见见世面”,大吃大喝后去了洗浴中心,可能喝多了的原因,糊涂得与洗浴中心的小姐发生了一次性关系,以后再未来往,也无其他类似经历。但内心在很长一段时间都非常后悔,后悔自己那次糊涂,并担心会因此染上艾滋病。为此我整天想着这件事,心烦意乱、紧张不安、失眠。于是上网了解有关艾滋病的知识,也曾偷偷给疾控中心打电话,知道自己得爱滋病的可能性非常小,但我仍很担心害怕,学习工作都无法集中注意力。

求助者自述情况:柳某神色慌张而羞怯,大步跨入咨询室后赶紧把门关上。柳某认为自己是个怪人,有害羞的怪毛病。两年多来,从不多与人讲话,与人讲话时不敢直视,眼睛躲闪,像做了亏心事。一说话脸就发烧,低头盯住脚尖。心怦怦跳,肌肉起鸡皮疙瘩,好像全身都在发抖。她不愿与班上同学接触,觉得别人讨厌自己,在别人眼中是个“怪人”。
最怕接触男生,即使在寝室里,只要有男生出现,也会不知所措。对老师也害怕,上课时,只有老师背对学生板书时才不紧张。只要老师面对学生,就不敢朝黑板方向看。常常因为紧张,对老师所讲的内容不知所云。更糟糕的是,现在在亲友、邻居面前说话也“不自然”了。

病人1980年结婚,婚后多年不孕,四处就医。1987年元月做诊断刮宫,术中无明显不适。但术后出现阴道流血。病人较恐慌,听同事说有癌症的可能,更加紧张,感心慌、气促。两周后,经处理出血停止,但病人仍恐怖,患有不治之症,又怕不能生育会被丈夫抛弃。诉失眠、烦躁、易激怒,对外界环境兴趣降低,但尚能坚持工作和操持家务。
三个月后症状加重,并出现发作性极端烦躁、坐卧不宁、呼吸急促、胸闷、心悸、出汗、手脚麻木。自觉会发疯、变傻,有濒死感,每次发作持续半个小时至1小时不等,几乎每天皆发作。发作间歇期仍有烦躁,担心再发,但尚能控制自己。至5月份,症状更加严重,无明显间歇期,整日处于惶恐不安之中,有明显的自杀企图,主要是因为“太难受了”,但同时又怕死。不愿去精神病院看病,爱人只好请医师至家中,病人抓住医师不放,反复叙述自己的病痛。经常无故找爱人吵闹,将自己的不适归咎于爱人不关心、不理解她。彻夜不眠,也不让爱人安睡,对爱人十分苛刻,要求他百依百顺,否则就大吵大闹。怕听收音机,怕看电视,怕人打扰,但周围无人又感到十分孤单。看到离婚或丧偶的女性触景生情,心中出现“一阵阵绞痛”。整日愁眉苦脸,喋喋不休,叫喊“不得了”。严重时撕头发,捶胸顿足,两手不住在腹部揉搓,致使上腹部皮肤发红、肿胀,有时甚至在地上打滚。至此,己完全丧失工作和操持家务的能力,生活难以白理。
8月份去某市精神病院就诊,当时诊断不详,给予多虑平、奋乃静、安定等药,10日后症状明显改善,且恢复工作。9月份,因小事与一丧偶女同事交谈,激发焦虑情绪,次日症状复发,表现同前,再用多虑平等治疗,无明显好转,且出现震颤、运动迟缓等副作用。
病中食欲增加,特别爱吃零食,甚至可以边哭边吃。既往素健,无明显躯体和精神疾病史。病前性格急躁,易激惹、好强固执、好攻击、心胸狭窄、多疑多虑、做事犹豫不决。
否认家系中有精神疾患和遗传性疾病史。
精神检查:意识清,仪表整,接触合作,焦虑、恐慌貌,两眉紧锁,不能保持安静,动作多,搓手顿足、来回走动。在交谈中,时时抓住医师的手,喋喋不休叙述白己的痛苦,声泪俱下,呻吟不止,反复讲“不得了”,“快救我”,语言罗嗦重复,认为“辛辛苦苦建立起来的家庭会被别人夺走”,否认有嫉妒妄想,后悔自己住院太晚,耽误了时间。自知力充分,求治心切。
心理诊断技能训练案例二
陈某,女,23岁,未婚,中专文化。因与人交往时紧张、局促不安三年,羞见生人、回避社交一年求治。
病人姊幼时家境不宽裕。父亲平日寡言,性格暴躁,动辄训斥。母亲懦弱厚道,逆来顺受。病人童年身心发育无异,懂事甚早,自诉入学前便颇知几分人情。文静、朴实,举止谨慎,懂礼貌、善自制,学习刻苦,成绩优良,常常因为某次考试未得第一名,或某次表扬没有自己,而怨别人或自责。19岁时进入某专科学院在花枝招展的女同学们面前颇感压抑,立志发愤读书,以示不随波逐流,但成绩平平,故时而气馁,悲观,时而愤愤不平,甚有嫉妒之意,然而在公开场合却又显得不以为然,落落大方。
常暗中注意同学们对自己的态度,如言语中有无含沙射影?眼神中有无蔑视怠慢?某日上课,发现新来的青年男教师似乎老是注意自己,颇难为情,课后照镜子观察,并未发现像貌装束有何刺眼之处,不禁浮想联翩,心慌脸红。迎面相逢,教师若无其事,病人却面红耳赤,心慌气促。眼神想主动回避,但又非看不可,越是回避,越是要看,心理矛盾,不可自拔。此后只要遇上该老师便言行失措。心情极度焦虑,担心老师有不良看法,惶惶不可终日,为此寝食不安。上课之前就心慌、气短、出汗,不敢出门。
久之,似觉同学们也洞察其心中隐秘,于是与同学们相处也不自然了。毕业后,父母告诫病人“听领导的话,守规矩,不要让人讲闲话”,等等,病人奉为信条,努力工作,上班时从不参与同事的清谈,有时同事主动与其交谈,也因不敢正视对方而假装做其他事,或托词走开。其办公桌面壁而放,背对同事,上、下班故意加速或放慢速度,避免与人同行。自知如此做实无必要,甚至会遭他人非议,但又不敢与人相处,常因此陷入矛盾之中。
后经人介绍结识一男友,才貌均颇中意,两人虽同在一市,却多是鸿雁往来。虽向往能像别的年轻人一样相互依偎在花前月下,挽手出入于剧院、商场,但每次下定决心,马上心慌、脸红,以致勇气全消,偶应邀赴约,也总是姗姗来迟。一次男方父亲寿辰,特邀病人赴宴,病人无词推托,几天前便开始忐忑不安,惶惶不可终日。赴宴那天,硬着头皮来到男方家中,未见公婆就心慌气促,四肢发颤到与公婆见面时,顿觉头昏目眩,满身大汗,语无伦次。众人误以为病中暑,急送医院部诊。
此后,几乎羞见一切外人,甚至与家人共餐也有些不自然。为避免这种窘境,常托词休病在家,以免交往。
体格检查及实验室检查无特殊阳性发现。精神状况检查未发现精神病性症状。明显焦虑,自诉“得了一种怿病,怕见人的病”。深知这种害怕毫无必要,称“没有见不得人的事”,但事到临头又紧张不己。病人对此百思不解,因而苦恼、不安,迫切求治。
心理诊断技能训练案例三
张某,男性,35岁,已婚,中专文化,某县小学教师。因失眠、精神差、易疲劳近4年而就诊于某院神经症专科门诊。
病人自诉4年前所在学校大抓教学质量,工作任务加重,觉压力很大。3个月后开始失眠表现为入睡困难,伴多梦易醒。躺下后辗转反侧,往往要二三个小时后才能入睡。睡后极易惊醒,轻微的响声都可使其醒来,睡中梦境频频,内容多与日间上作有关。白天病人侧感到昏昏欲睡,精力极差,易感疲劳,连上4楼也感到吃力。工作能力受到严重影响,有时甚至不能坚持上完一节课,病人自感只有躺在床上才略觉舒服些。
曾到当地医院内科、神经科就诊,多项临床检查未发现任何躯体疾病,多次被诊断为“神经官能症”、“神经衰弱”、“阴虚”等症。曾服用许多中西药物治疗,疗效甚微。近半年来病人到某省属师范学校校长培训班学习,更感难以胜任,上课时注意力不能集中,有时一堂课下来。竟不明上课所讲的主要内容。自感记忆力和学习效率下降,一看书便心烦意乱,对所读内容印象不深,记忆力下降到有时连非常熟悉的同学的名字也叫不出。近3个月来开始头昏、头痛,伴眼花,但无恶心、呕吐。情绪急躁、不快,并常因小事叹息不已,遇高兴事可暂时有所好转。起病后食欲尚可,体重无明显变化。
既往体健,否认慢性躯体疾病或传染病史,否认各种精神疾病史,亦无颅脑外伤、颅内感染或高热昏迷史。
出身于职师家庭,童年时父母教育较为严格,在校学习成绩及在单位工作能力均属中等。病前性格以多疑、敏感、不果断、易急躁、自信心不足及情绪不稳定较为突出家庭中除其父亲有长期失眠病史外,否认两系三代成师有精神病史或遗传性躯体疾病史。体格检查:未发现阳性体征。
精神状况检查:接触良好,检查合作,外貌显萎靡。自觉非常敏感。无幻觉、视物变形等感知觉障碍。表述清楚,未发现思维逻辑障碍,语速正常未发现妄想。情绪较低沉,认为自己的疾病非常顽固,难以治愈因而甚为着急。病人认为,只要治好现患疾病,自己将会有较好的前途。无明显悲观厌世情绪。检查过程中病人注意力能集中,近期与远期记忆正常。自知力存在,主动诉述病情,求治心切。
实验室检查:三大常规、肝肾功能、心电图、甲状腺功能检查未发现异常
心理诊断技能训练案例四
王某,男,27岁,工人,未婚。因孤僻懒散,与人长期格格不入,流浪街头数年于1972年9月被送来院就医。
1968年分配到土产公司工作时即发现病人孤僻,不与人来往,分配工作任务完成不好,甚至不干,好计较而与人争吵,曾搬迁数次,认为公司里大学生能住较好的房子自己为什么不能住,将自己住的房间门窗拆下,壁纸撕掉,搬到大礼堂去住,到处找领导要房子、工资,不上班,将自己的东西拍卖。逐渐发展到流浪街头,睡门洞,到餐馆讨饭吃,时常叫喊母亲的名字(母已自缢死亡)。在马路中间走,认为汽车不敢撞他,偶有自语。病人为遗腹子寄居外祖母家,母亲因患精神病及下肢瘫痪自杀死亡。
小学成绩好,1964年因自然灾害,家庭经济困难而失学。性格喜静,常独居,不愿结识他人,母去世后,性情更加孤僻。不顺心时怒气大,浑身发抖,曾任生产小队会计,1966年参军,因“精神不正常”而住院8个月左右,1968年复员进土产公司。
体格检查:头颈胸未见特殊,心肺正常,腹平软,肝脾不肿大,脊柱四肢正常,神经系统检查无特殊。
精神状况检查:意识清晰,衣饰不甚整齐,对周围不主动接触,被动接触可,生活能自理,否认有病。自述个性较固执,不适应群体,自觉心烦,仍纠缠房子及工资等事。诉说常看到死去的母亲,想自杀。在整个检查中未发现思维散漫,但强词夺理。嘱其将感想写下来时,内容冗长,不时引证哲学词汇,常离题发挥,重复,繁简失当,如谈到与干部住房对比不公平问题上,认为“不能利用出现的一种倾向掩盖另一种倾向,也就是特殊情况特殊对待的问题,犯绝对平均主义的问题”。认为领导和周围的人对他不好,常感孤独,要把东西卖掉去找其母,对人冷谈,不谈笑,独来独往,要求出院。
