颐年园养老公寓协议书

根据老人公寓目前的条件,开办自费入寓颐养义务,为明确双方的权利义务关系,签订此协议。

甲方:张家口市颐年园老年公寓

地址:联系电话:。协议条款如下:

一、甲方同意接受乙方送来自费养老人员同志。

二、乙方作为自费养老人员经济责任人,丙方作为担保人应各自提供本人身份证及联系方式的证明手续,同时交押金两千元。

三、为了提倡全社会都关心支持福利事业,乙方必须向甲方每年按时缴纳各类颐养费用(床位费、日常生活护理费、卫生费、餐费等)共计元/月,大写金额:(),如果老人中途退房,(餐费按10元/天)只退餐费,护理费不与退还。

四、冬季取暖费每床每月元。

五、入住公寓的老人有下列情况之一者,甲方有权将其退回,同时终止协议履行。

1、发现有传染病或精神病患者;

2、不按期缴纳各种费用者;

3、若老人生病通知乙、丙方,而无任何一方来着;

4、不能遵守甲方各项规章制度、妨碍他人入住、甲方认为无法为其管理者。

六、老人入住公寓时,乙方自带被套、床单、枕套及其它用品。

七、老人在公寓养老期间外出需办理请假手续,若擅自外出不归,失踪或途中发生意外,后果自负,甲方均不负责。

八、老人病重、病危时甲方负责及时通知乙方或丙方,乙、丙方应在接到通知一小时内到寓,如在乙、丙方未到达前发生死亡,与甲方无关。老人后事均由乙、丙方负责在六小时内处理完毕。否则甲方将通知殡仪馆处理,甲方不承担责任。

九、甲方根据乙方体检情况并通知乙方离开公寓须到医院救治,而乙方没去的,出现的后果乙方自负。

十、乙方如发现身体不适应及时向甲方工作人员或医护人员反映,如因自身隐瞒而延误治疗,后果自负。

十一、凡入寓时身体状况和年龄一确定院内规定的护理标准,如老人在住寓期间由于身体状况或年龄超过入寓暂定的护理标准,甲方可提高护理标准,乙方按相应的标准交费。

十二、入住公寓的老人在日常生活中自行行动发生跌伤或自身行为造成的伤害和死亡者,由其自行承担责任。与甲方无关。

十三、乙方不服从甲方管理、不听甲方劝阻或不接受甲方服务、食用在外自购食品或探视亲友送来的食品等原因造成的损害,由乙方承担。

十四、医疗方面:

1、凡入住公寓的老人在入寓前需自行健康体验。

2、入住公寓老人需提供准确可靠病史,入住后发现因隐瞒病历而耽误治疗,其后果自负。公寓老人如有常见病、多发病,须治疗者可在公寓医疗室就诊,医疗费自理。

3、住寓老人如突发疾病、危、重病或猝死,公寓医务人员本着就死扶伤的精神,在及时通知乙方的同时将给予紧急处理和进行必要的抢救,所发生的医疗费,由乙方负责,如乙方已到达之前发生死亡与甲方无关,老人后事均有乙、丙负责在1小时内处理完毕。不能在公寓大操大办,否则甲方将通知殡仪馆来处理,甲方不承担责任。

3、住寓老人如突发急、危、重病时,导致增加的陪护,护理费用均由乙方负责。

十五、根据国家物价上涨或下跌的幅度,甲方可齐整入寓老人的收费标准。

十六、入住的自理老人的贵重物品及钱物由本人自行妥善保管,如发生丢失、被窃,甲方概不负责。

十七、本协议需入寓老人、甲、乙、丙三方签章并遵照协议条款办妥有关手续后生效。本协议一式三份,三方各执一份。

颐 和 园

啊!今天真是个好天气,我和姨姨、妹妹一大早起来,就匆匆坐车飞驶向颐和园。

进入颐和园,也像进入了江南仙境,一幅江南风光图立刻映入了我的眼瞳。一条犹如闹市般的苏州街蹦了出来,我走入了苏州街,有卖纪念品的;有正在算命的;也有正在展示玩具的。叫卖声此起彼伏。一条翠绿的小河把苏州街分成了两边,几座形态各异的汉白玉桥又不慌不忙地架了出来,一朵朵荷花,一片片荷叶竞相开放,一两条小船悠闲地停在岸边,任凭水波轻推着它左摇右摆。真似小桥流水人家呀!不难想像当时皇帝、太后在此游玩时有多么惬意呀!

我离开这美如画卷的苏州街,顺着一条由石子拼成的小道,走入了万寿山。周围松柏成群,忽然包围了我们,里面把阳光封得严严实实,周围的一切都发生了变化,就像一下进入原始森林一样。这里静静的,没有故宫那样一环环的建筑,也没有一群又一群的人。我异常兴奋,觉得这不是园林,而是山,山上有无数分叉路口,有被游人踩出来的,有专门修造的。万寿山并不像别的山,高而陡或平而缓,游人如织,树木不能像帐棚一样完全罩住人,处在城市中,空气也不清新。而这万寿山,道路由你选择,层层叠叠,或平或陡,自己仿佛是一个探路者,全身热血沸腾。山中空气清新,夹杂着山的味道、树的味道、自然的味道,使人心旷神怡,在一个叉路口,偶尔还能碰到几个正登山的人,山中,时不时有几个小亭子、小院,我们累了,上面一坐,说一说话,听一听各种鸟鸣啼啭,树上也时不时飞出几只鸟,大的、小的、白的、花的,它们看也不看我们一下自顾自地往远飞。路上时不时窜出一只小松鼠,毛茸茸的,可还没等我和妹妹惊呼,它已经没了身影。

走着走着,视野开阔起来,忽然,一个休息小亭出现了。这个亭子造工精巧,大体仿照苏州街,是一个圆形、中间有水,两岸架桥,水中还有几朵荷花,十分漂亮,真是柳暗花明又一村呀!此亭子背有靠山,中有滚滚流水财源,用意甚远!

走出这个亭子,就来到了万寿山上的精华建筑群。首先映入眼帘的是高高的佛香阁。这佛香阁黄璃瓦盖成的屋顶、对称式的建筑形态,精致的做工和顶上的几只小兽都象征着它无比高尚的地位。走进佛香阁,里面有一尊巨大的千手观音,所有的人一看都会心生敬畏,让人自觉得想顶礼朝拜。同时,千手观音还起着警告人们不要心生邪念,诚实做人。以这座高高矗立在万寿山上的佛香阁为中心,密密麻麻的小殿、戏楼、院亭都众星拱月般环绕着佛香阁,环环相接、造工精巧、天衣无缝!

万寿山不仅有一大群建筑精髓,还有一条家喻户晓的长廊,从万寿山一直连到了昆明湖,有几百米长,更是精品中的精品。长廊里有成百上千幅画的图,飞禽走兽等,个个栩栩如生,如果坐在长廊的旁侧,欣赏着这顶上形态各异的绘画,与其说是休息,不如说是在长廊博物馆里欣赏文物。连妹妹都说:“太后和皇上太会享受了!”是啊,光建长廊要多少人力、物力和时间,更别说整个颐和园了,真是让人眼界、心界、脑界三界大开呀!

顺着长廊往下走,眼前,突然出现了一个淡白的世界,我以为自己看错了,再仔细一瞧,不错,昆明湖!我高兴地又蹦又跳。放眼望去,广阔无边,各种各样的船停泊在湖中,犹如蓝白相间的绸子上绣了一朵朵五颜六色的花,真是“船水艇轮”呀!一条白色的孔桥架在湖中,数上一数,有十七个孔,啊!原来这就是那大名鼎鼎的十七孔桥!桥一直通向了南湖岛。走上了桥,看着这一个个形态各异的小狮子仿佛在仔细瞧我。我靠在桥边,感受着一丝清凉的微风,朝远望去,一切都披上了纱一样的雾,朦朦胧胧,近处的水在轻轻地、带着节奏地摇摆着,岸边的柳条长长地垂了下来,有一小截垂入水中,水太顽皮了,一摆动,柳枝也微微一晃,真是无风自动。眼前的一切都是那么美妙绝伦,无法描写。如果说长城的气息是宏伟雄壮、忽高忽低、奔腾万里、让人热血沸腾;那这昆明湖就是温柔、轻飘飘的如纱一般把你带到另一个世界,同时让你感到一种广阔和无边......“轰--轰”老天爷的脸终究是沉了下来。雷声大作,瓢沷大雨如天神倒下来的一缸水,越下越狠。我们赶忙躲在一个观赏亭下,过了一会,雨小了,颐和园在它那被雨水洗刷了上万次的记录上又加了一回,一切又变得倍加清新。姨姨不敢停留,带我们冒着小雨匆匆离开了,望着远处若隐若现的颐和园,我和妹妹都依依不舍,直到颐和园消失在远处,我朝那里投下了最后一瞥,心里涌出一种前所未有的坚定,颐和园!

甲方:合川区九九养老公寓

乙方:监护人:

根据《中华人民共和国合同法》,本着公平自愿的原则,经甲乙双方协商,达成如下协议。

一、甲方提供养老居住的条件:

1、房间有独立卫生间洗簌间;公用卫生间、洗簌间;公用卫生间、洗漱间、客厅的套间等供入住老人选择。卫生间配有马桶、淋浴、浴霸等设施。房间配有空调、电视、衣柜、床被床单等。

2、休闲、娱乐、健身场所,室内大厅(兼餐厅)120平方米,室外场地100多平方米。另设有文化阅读室、伙房、办公等场地。

3、饮食安排,根据老年人身体的特殊性,按不同季节合理安排一日三餐的生活,经常征求老年人意见,制定食谱。亲属亲友可订餐包桌席。

4、健康医疗。甲方与合阳城社区卫生服务中心(本公寓街对门)合作,由其提供医疗诊治,定期体检及健康档案的建立等医疗保障。如有需要就医的,甲方应积极送护诊治,所发生的一切医疗费用,全额由乙方自行承担。

5、管理,甲方根据行业要求,合理设置工作岗位,建立建全各项岗位责任制和管理制度,确保入住老年人在本公寓居住期间生活舒适愉快。

6、经常与乙方(监护人)交流情况,让入住老年人住的安心,使其监护人放心。监护人的联系电话变更时应及时告知甲方。

二、乙方入住应具备的条件;

1、乙方应对本公寓进行实地考察,并对入住须知、入住协议、管理制度、服务水准及收费情况进行全面的了解后,老年人和监护人协商、意见一致,自愿入住。

2、入住老年人的身体较为健康,思维清晰正常,行动自如,可自行料理本人日常生活。能与其他老年人正常相处,喜欢集体生活。

3、入住前须经合川区级医院体检,证明无传染病、精神方面的疾病,或其他不宜集体生活居住的疾病。

4、有一定的经济能力,能按期足额缴纳入住费用。监护人能为老人承担监护的义务和责任,定期探望老人,对其进行必要的精神安慰。

三、入住费用,每人每月入住费用为元,乙方可选择年、半年、季度、月交费等缴费方式,每月25日前必须一次性缴清下月费用,不得拖延。其费用组成为、床位费、生活费、水、电、气、洗理、健身娱乐等费用。

四、医疗费用;乙方生病所发生的一切费用由乙方自行承担。需要住院、就诊的,甲方及时通知其监护人,由监护人负责办理相关手续并陪护。入住时,乙方同意向甲方交医疗备用金元,离退本公寓时,按实际余额退还。

五、安全防护

1、甲方应加强对乙方的安全意识提示,加强安全管理,设置必

要的安全标识、警示牌等,防火防盗防意外事故的发生。

2、乙方应严格遵守本公寓的各项规章制度,增强安全意识。

(1)贵重物品应妥善保管,也可交由甲方代管(免费)。由乙方自己保管而发生遗失、被盗等问题的,由乙方自行负责;

(2)禁止在卧室内玩火、酗酒或趟在床上吸烟。并正确使用各种电器设备。如因乙方因素造成火灾或其他事故,均由乙方承担全部责任。

(3)在日常生活中应注意安全,上下楼梯时,应手扶扶手,小心慢行。如不慎发生意外受伤,甲方及时送医,由此所发生的一切费用由乙方承担,甲方不承担任何责任。

(4)入住老年人因病身故,或因其他因素导致的自虐、自残、自杀等行为经抢救无效身故的,甲方不承担任何责任。且由乙方承担由此所发生的一切费用。
