医疗工作制度
一、医生必须以救死扶伤、防病治病、为公民的健康服务为宗旨。
二、医生上岗要着装整洁;使用文明用语;关心体贴病人;态度和蔼有礼貌;耐心地解答问 题;有计划地安排病人就诊。
三、对病人要认真检查,做好登记,实行首诊负责制。
四、医生应严格按照诊疗常规操作,门诊病历必须认真填写,首诊病历要求有主诉、重要病 史、既往史、各种阳性体征、必要的阴性体征、诊断或印象诊断、治疗及处理意见、医 生签字。复诊病历要求有主诉、体检、诊断、处理、签名。
五、医生要采用保证疗效、经济便宜的治疗方法,科学用药、合理用药,尽量减轻病人负担。
六、认真执行病历处方书写,做好急救危重病人的抢救工作。如有未明确诊断的,应介绍到 上级医院,病情危重需转院的,应护送到上级医院。
护理工作制度一、二、三、四、五、六、七、八、护士上岗要衣帽整洁、戴口罩。认真执行查对制度,保证护理安全。严格执行各项护理技术操作规程。严格执行预防交叉感染的规定。严格执行疾病护理文件。认真书写各种护理文件。护士要经常巡视病房,密切观察患者的病情变化,如有危重患者,要做 好抢救工作,需转院治疗的要护送到上级医院。处置室内清洁区、污染区要分开,标志鲜明,有菌物品与无菌物品应分 开放置,并设无菌物品专柜。
消毒隔离制度
一、必须备有高压灭菌设备,有监测记录。
二、治疗室、处置室要定时进行紫外线照射消毒,周末进行总体消毒处理,并做好记录。
三、无菌柜内物品必须做到无菌,每种物品必须记录好消毒、灭菌时间。
四、清洁区、污染区必须严格分开。
五、病人呕吐物、排泄物(大小便、痰)及便器等,应专项严格消毒。
六、一次性用品不得重复使用,做到一人、一针、一管、一带、一巾、一用、一消毒。
七、对就诊的肠道病人,应向其做好卫生宣传工作,发给消毒药物,并指导消毒办法。
八、对已确诊或疑似为传染病的病人,应按传染病管理办法及时同有关部门联系,上报或转送到相关传染病医院进行诊治。
消毒培训制度
一、门诊部医务人员人人重视消毒灭菌工作,做到操作中避免感染源传播。
二、定期参加区市消毒培训,本门诊部一月一次总结消毒工作。
三、新来人员必须先行消毒知识学习,经考试合格后方可上岗。
四、门诊设有消毒监督检查专职人员,随时检查监督,消毒工作的每一步操作环节,发现问题随时汇报,及时纠正。
五、定期请卫生监督部门前来检查指导工作。
疫情报告管理制度
1、执行职务的医疗保健人员、卫生防疫人员、个体开业医生为责任疫情报告人。责任疫情报告人发现法定传染病病人、疑似病人、病原携带者应在规定的时限内,向区卫生防疫站报告,必要时做订正死亡报告。
2、各医疗卫生机构及人员要认真学习《传染病防治法》、《突发公共卫生事件应急处理条例》等法律、法规和传染病防治知识,熟练掌握传染病诊断、报告、隔离消毒及疫情处理的程序,切实增强传染疫情报告意识,发现传染病例要认真做好传染病登记,填写传染病报告卡,在规定时限内向卫生防疫机构报告。
3、在各医疗机构防保科设立传染病疫情报告员。传染病报告员接到临床医生的传染病报告要进行核实诊断,认真做好记录,并及时上报当地卫生防疫机构。认真开展疫情主动监测工作,对临床有关科室的门诊日志和住院登记至少每旬开展一次疫情搜查,发现漏报的传染病病例,要及时进行补报。
4、责任疫情报告人发现甲类传染病、传染性非典型肺炎和乙类传染病中的艾滋病、肺炭疽的病人、脊髓灰质炎的病人、病原携带者或疑似病人,城镇于2小时内,农村于6小时内通过传染病疫情监测信息系统进行报告。对其他乙类传染病人,疑似病人和伤寒、副伤寒、痢疾、梅毒、淋病、乙型肝炎、白喉、疟疾的病原携带者,城镇于6小时内,农村于12小时内通过传染病疫情监测信息进行报告。对丙类传染病和其它传染病,应当在24小时内通过传染疫情监测信息系统进行报告。
5、传染病暴发、流行时,责任疫情报告人应当以最快的通讯方式向当地卫生防疫机构报告疫情。接到疫情报告的卫生防疫机构应当以最快的通讯方式报告上级卫生防疫机构和当地政府卫生行政部门,卫生行政部门接到报告后,应当立即报告当地政府。
6、流动人员中的传染病人,病原携带者和疑似传染病人的传染病报告、处理由诊治地负责,其疫情登记、统计由户口所在地负责。
7、区卫生防疫机构应当对辖区内种类医疗保健机构的疫情登记报告和管理情况定期进行核实、检查、指导。
8、任何单位和个人对突发事件和传染病疫情,不得隐瞒、缓报、谎报或者授意他人隐瞒、缓报、谎报,否则将依法追究责任。
9、任何单位和个人发现传染性非典型肺炎病人或者疑似传染性非典型肺炎病人时,必须2小时内以最快的方式向区卫生防疫站报告。区卫生防疫站发现疫情或者接到疫情报告,应当立即报告上级防疫机构和当地卫生行政部门。
10、区卫生防疫站疫情管理人员应定期编发疫情简报,对重大疫情进行预测、预报。
传染病病例处置工作制度
制订传染病预防控制工作方案和重大疫情应急处置技术方案,组建传染病疫情应急处置队伍,做好传染病诊断试剂、生物制品、防治药品、消毒除害药械等的应急储备工作。认真落实传染病疫情的预测预警措施和专报制度,加强信息传递和沟通,及时发现传染病疫情苗头,一旦在重大活动参加人员中出现传染病疫情,必须迅速启动应急预案,采取果断措施,严防疫情扩散与蔓延。
(一)预防为主
坚持“预防为主”的卫生工作方针,按照“早发现、早诊断、早隔离、早治疗”的传染病防治原则,提高警惕,加强监测,及时发现病例,采取有效的预防与治疗措施,切断传播途径,迅速控制重大疫病在我市的传播和蔓延。
(二)依法防治
为有效切断传染病的传播,根据有关法律、法规、结合重大疫病的流行特征,在采取预防控制措施时,对留院观察病例、疑似病例、临床诊断病例及实验室确认病例依法隔离治疗,对疑似病例、临床诊断病例及实验室确诊病例的密切接触者依法实行隔离和医学观察。
(三)开展医疗救治
按照医疗救治方案启动指定的专门医疗救治网络,开展病人接诊、收治和转运工作,实行重症和普通病人分别管理,对疑似患者及时排除或确诊。做好消毒隔离、个人防护和医疗垃圾处理工作,防止院内交叉感染和污染。并参与组成专家组负责全市重大疫病的会诊工作。
预防和控制重大疫情要坚持“早、小、严、实”的方针,对留院观察病例、疑似病例、临床诊断病例及实验室确诊病例,要做到“及时发现、及时报告、及时治疗、及时控制”。同时,对疑似病例、临床诊断病例及实验室确诊病例的密切接触者要及时采取隔离控制措施,做到统一、有序、快速、高效。
医疗废物管理制度
一、医疗废弃物必须与生活垃圾分类管理。
二、按《医疗废物管理条例》规定要求建立医疗废弃物专用的密封贮存室,标明标志并加锁,防止医疗废弃物的流失,泄漏和扩散。
三、由门诊指定专人对输液管和注射器类(经过消毒、毁形)、玻璃器皿类、组织液样本、试管或其它类进行分类,分别用易燃无毒的软包装袋密封装好,并贴上分类标识卡。
四、分类包装好的医疗废弃物由清洁工送至存入医疗废弃物的贮存室的专用密封桶中。
五、定期由后勤人员将医疗废物送至县卫生局指定地点,针头等类物品定期焚烧处理。并如
实填写收运交接时间、种类、数量的医疗废弃物转移联单和经办人签名。
六、医疗废弃物贮存室每天由清洁工清洗,并用2500mg/L的含氯消毒剂喷洒消毒。
七、对有关接触处理医疗废弃物的人员定期进行知识培训。

口令:立正。
要领:两脚跟靠拢并齐,两脚尖向外分开约60度;两腿挺直;小腹微收,自然挺胸;上体正直,微向前倾;两肩要平,稍向后张;两臂自然下垂,手指并拢自然微屈,拇指尖贴于食指的第二节,中指贴于裤缝;头要正,颈要直,口要闭,下颌微收,两眼向前平视 口令:跨立。
要领:左脚向左跨出约一脚之长,两腿自然伸直,上体保持立正姿势,身体重心落于两脚之间。而手后背,左手握右手腕,右手手指并扰自然弯屈,手心向后。口令:稍息。
要领:左脚顺脚尖方向伸出约全脚的三分之二,两腿自然伸直,上体保持立正姿势身体重心大部分落于右脚。口令:向右(左)--转
要领:以右(左)脚跟为轴,右(左)脚跟和左(右)脚掌前部同时用力,使身体和脚一致向右(左)转90度,体重落在右(左)脚,左(右)脚取捷径迅速靠拢右(左)脚,成立正姿势。转动和靠脚时,两腿挺直,上身保持立正姿势。口令:向后--转。
要领:按向右转的要领向后转180度。口令:齐步--走。
要领:左脚向正前方迈出约75厘米着地,身体重心前移,右脚照此法动作;上体正直,微向前倾;手指轻轻握拢,拇指贴于食指第二节;两臂前后自然摆动,向前摆臂时,肘部弯屈,小臂自然向里合,手心向内稍向下,拇指根部对正衣扣线,并与最下方衣扣同高 口令:立--定。
要领:齐步和正步时,听到口令,左脚再向前大半步着地,两腿挺直,右脚取捷径迅速靠拢左脚,成立正姿势。口令:蹲下。
要领:右脚后退半步,臀部坐在右脚跟上(膝盖不着地),两手自然放在两膝上,上体保持正直。蹲下过久,可自行换脚。口令:起立。
要领:全身协力迅速起立,成立正姿势。
