1.上海志愿者证功能介绍

1、在本市范围内跨组织、跨地区综合服务记录；

2、由志愿者组织完成志愿者参与志愿服务的网上记录和手工记录； 3、20万元志愿服务意外保险，50元/天住院补助。

2.如何申领

1、个人可向所在志愿者组织申领志愿者证。

2、如没加入志愿者组织，可点击志愿服务项目列表，选择合适你的项目加入，经项目组织方审核同意后，即成为注册志愿者。

3.如何记录志愿服务时间

3.1网上记录

1、志愿者组织直接把志愿服务信息输入上海志愿者管理系统。

2、通过志愿服务记录器

（1）志愿者组织先为志愿者开通《上海志愿者证》；

（2）志愿者组织在志愿服务记录器上刷《上海志愿者证》芯片，记录志愿服务信息并上传至上海志愿者管理系统。

3.2《上海志愿者证》记录

1、志愿者组织在《上海志愿者证》上记录志愿服务信息。

2、志愿者可要求所在志愿者组织把志愿者证上的记录录入网上管理系统。

4.如何查询志愿服务记录及完善志愿者个人信息

1、登陆上海志愿者网（下载）； 《理赔申请书》网上下载说明：

登录“中国人寿”网站

依次点击“客户服务”-“保单理赔服务”

在网页右下方可见“相关单证下载”-“理赔申请书”，即可下载使用。

2、《保险证明单》（登录上海志愿者网 【公告栏】下载）； 保险证明单（右键保存）

3、申请人有效身份证件复印件（正反面）；

4、病历和出院小结（原件,包括各类化验报告、CT、X光片等）；

5、住院医药费收据和住院明细清单（原件）；

6、受益人的银行帐户（复印件，须有持卡人清晰的签名）；

7、如导致残疾，还需提供以下资料的原件：

（1）首诊病史及自发生保险事故时到180天时的病史资料；

（2）区（县）级以上医院或鉴定机构出具的被保险人残疾程度的资料或

身体残疾程度鉴定书。

8、如导致死亡，还需提供以下资料：

（1）区（县）级以上医疗机构出具的《居民死亡医学证明书》、《居民死亡推断书》或公安机关出具的《居民死亡确认书》。交通事故应同时提供《道路交通事故认定书》。如为驾驶员还须提供有效的驾驶证和行驶证；

（2）经公安机关注销的被保险人的户籍证明。

5.4理赔流程

1、遇险后7天内，请拨打中国人寿客服电话 95519报案；

2、发生事故后，被保险人请至二级以上（含二级）医院就诊；

3、诊疗结束后由志愿者所在志愿者组织将理赔材料交至区志愿者协会、总队，由区志愿者协会、总队审核敲章后统一交由市志愿者协会；

4、市志愿者协会审核后交由中国人寿保险公司；

5、中国人寿保险股份有限公司根据保险条款，审核并实施赔付。

第二篇：志愿者服务证

关于制作“志愿服务证”以及“校团委志工部公章”的申请 尊敬的四川农业大学校团委：

目前川农志愿者活动开展地如火如荼，并取得了一定社会效应与成果。然而对于志愿者身份认定的问题仍然困扰着广大的志愿者们：他们自己志愿者的身份没有办法被外界确认，自己的志愿服务没有东西可以作为统一的凭证。这样随着志愿者归属感及荣誉感的减弱，很多人在大一结束之后就退出了志愿者的行列，而这是我们非常不愿意见到的。

所以，为了适应当下川农志愿者服务活动不断大型化、规律化、高质化的发展趋势，我们在此申请制作“志愿服务证”以及“校团委志工部公章”。这样做的目的在于:

第一,让川农志愿者从此有了“身份证”，增强志愿者的归属感;

第二,让志愿者参加的活动从数量到内容变得有凭有据，增加志愿者的荣誉感;

第三,便于组织志愿服务的单位对于参与服务的志愿者进行统一管理;第四,通过“志愿服务证”，让社会上更多的人了解志愿者，促进志愿服务的顺利进行.同时我部门也会加强对志愿者注册的把关,并设立一系列公章使用及管理制 度，相关制度见附件一。

1.志愿者服务证由四川农业大学校团委统一制发，并由志愿者所在组织进行管理。

2.志愿者服务证只作为志愿服务记录之用，不作其他用途使用。

3.志愿者服务证由志愿者使用及妥善保管，不得转借、冒用或不当使用。4.对因违反章程被劝退或开除的志愿者，其所在的注册机构应收回服务证。5.志愿者服务证变更志愿者服务组织时，服务证继续使用。6.志愿者每年需在所在志愿者组织进行注册登记。

7.服务证如损坏、损失或写满，志愿者可申请补发、续发志愿服务证。8.填写志愿服务证记录应注意：

如实填写服务记录；服务内容应简略填写此次服务的名称等；服务时间是指实际服务的时间段（不含往返途中时间）；每项服务记录要有服务对象或组织者签署姓名方为有效。

第三篇：上海世博会志愿者

中国2010年上海世博会志愿者标志的主体由汉字“心”、英文字母“V”、嘴衔橄榄枝飞翔的和平鸽构成。与世博会会徽“世”异曲同工，在呈现中国文化个性的同时，表达了志愿者的用“心”和热“心”。“V”是英文“Volunteer”的首字母，阐述了标志所代表的群体，赋予其清晰的含义；飞翔的和平鸽代表上海，也象征和平友爱，橄榄枝则寓意可持续发展和希望，传承“城市，让生活更美好”的世博会主题。彩虹般的色彩，迎风飘舞的彩带，是上海热情的召唤。

中国2010年上海世博会志愿服务工作常见问题解答 1.什么是中国2010年上海世博会志愿者（以下简称世博会志愿者）？ 中国2010年上海世博会志愿者，是指由中国2010年上海世博会志愿者工作组（以下简称组织方）统一招募的，在上海世博会园区内外自愿无偿地开展各类志愿服务活动的所有志愿者。

2.世博会志愿者共分为几类？ 世博会志愿者分为两大类：一类是为世博会园区提供志愿服务工作的世博会园区志愿者；一类是在整个城市开展各类志愿服务工作的世博会城市文明志愿者，其中包括为城市文明活动提供志愿服务工作的世博会城市志愿服务站志愿者。本次招募的志愿者主要是世博会园区志愿者和世博会城市文明志愿者中的世博会城市志愿服务站志愿者。

关于申请志愿者服务证的通知

为了更好的证明自己成长过程中的足迹，同时进一步推进我校志愿者行动向规范化、项目化、阵容化方向发展，现全面办理10、11级志愿者服务证。

申请志愿者服务证要求：

1、本学期内志愿服务次数达到4次及以上；

2、申请时申请书请写清个人院系、班级、姓名、联系方式；志愿服务活动地点、名称、时间及负责人；近期免冠彩照一张。

3、如果符合以上两个要求的同学可写好申请书交至志愿者管理中心办公室（1506）。

注：需要服务证填写的(已有服务证同学)请注明志愿服务活动地点、名称、时间及负责人，连同服务证交至志愿者管理中心办公室（1506）。

志愿服务证用于证明注册志愿者的身份，记录注册志愿者参加志愿服务的时间、内容和所获的志愿服务荣誉。请大家慎重对待!
