评审材料有关问题的说明
为了使评审材料报送、填写真实规范,确保职称评审工作顺利进行,现就我市申报高、中级专业技术职务资格,报送评审材料过程中,有关具体问题说明如下:
一、申报人须提供的评审材料类别、数量
1.《山东省专业技术职称评审表》1份(A4纸型,原件,双面打印,左侧装订)。
2.《专业技术职称评审简表》一式3份(A4纸型,原件,双面打印)。
3.《专业技术人员申报评审职称情况一览表》一式3份(A3纸型,原件),须使用《山东省职称评审管理系统》打印生成。
4.《济南市推荐评审高、中级专业技术职务任职资格量化赋分表》1份(A4纸型,原件)。
5.反映本人任现职以来专业技术水平、能力、业绩的业务工作总结1份;代表性著作、论文、作品,成果及奖励证书等(原件)。其中,《专业技术人员申报评审职称情况一览表》中“任现职以来取得的成果及受奖情况”(以下简称 1
“成果及受奖”)栏填报数量不超过5件;“任现职以来发表、出版的论文、著作、作品等”(以下简称“论文、著作、作品”)栏填报数量不超过5件,其中代表性著作不超过2部。填写《专业技术人员申报评审职称情况一览表》时,“成果及受奖”栏,应按要求填写“核心提纲、解决问题、效果”等内容;“论文、著作、作品”栏,应按要求填写“刊号、提纲、观点、文章字数”等内容。

二、申报人所在单位、主管部门、呈报部门应履行的手续及要求
1.所在单位、主管部门、呈报部门要对申报材料认真审查,严格把关,经公示无异议的,申报人、审核人、单位负责人分别在申报材料的相应意见栏中签字,加盖印鉴后,连同其他申报材料,由呈报部门按规定时间报送到指定地点。
2.符合破格申报专业技术职务任职资格的,须由单位和有关主管部门出具破格推荐报告1份,并由呈报部门核准签署意见,加盖印鉴。
3.呈报部门对申报材料审核后,同时上报通过《山东省职称评审管理系统》生成的数据汇总盘1份。
4.呈报部门填报《专业技术职务任职资格申报人员花名册》2份(A4纸型,原件,加盖印鉴,附数据盘)。
三、其他要求
1.呈报的申报材料必须手续完备,内容齐全,字迹清晰,不得涂改、漏页、缺页。
2.申报材料须按要求装入规范的材料袋内,实际材料件数与材料目录必须相符。
3.各级人事部门要严格按照规范的程序,认真组织好申报专业技术职务任职资格的评议推荐,申报材料的填报、审查、审核、呈报工作,确保提供的评审材料真实、准确、有效。报送省里评审的,按省各评委会办事机构有关要求办理。
